睑板腺功能障碍致蒸发过强型干眼症的临床护理方法及效果 被引量:2

摘要 目的探讨睑板腺功能障碍(MGD)致蒸发过强型干眼症(DED)的临床护理方法及效果。方法将1089例MGD致蒸发过强型DED患者分为观察组545例与对照组544例,对照组采用常规护理方法进行干预,观察组在对照组基础上加行眼睑热敷、清洁与按摩以及从生活、心理、饮食等多方面的护理干预,比较两组患者护理干预前及干预后1、2、4、8周的基础泪液分泌试验(SIt)、泪膜破裂时间(BUT)、角膜荧光素染色(CFS)及睑缘形态异常评分(LMS评分)。结果自干预1周始,与对照组比较,观察组基础泪液分泌量显著更多,泪膜破裂时间显著更长,角膜荧光素染色评分与LMS评分显著更低,组间比较均有统计学意义(P<0.05或P<0.001)。结论有效的护理干预可明显增加基础泪液分泌量、改善泪膜脂质层功能、促进角膜与眼表损害修复及睑板腺分泌功能的恢复,继而提高MGD致蒸发过强型DED的临床治疗效果。 Objective To explore the clinical nursing method and effect of DED caused by MGD.Methods 1089 cases of DED caused by MGD were divided into observation group(545 cases)and control group(544 cases).The control group was given routine nursing intervention.The observation group was given eyelid hot compress,cleaning and massage as well as nursing intervention from life,psychology and diet on the basis of the control group.The basic tear of the two groups before and 1,2,4 and 8 weeks after intervention were compared.Secretion test(SIt),tear film rupture time(BUT),corneal fluorescein staining(CFS)and eyelid margin abnormality score(LMS score).Results From the first week of intervention,compared with the control group,the basic tear secretion of the observation group was significantly more,the tear film rupture time was significantly longer,the corneal fluorescein staining score and LMS score were significantly lower,and there was statistical significance between the two groups(P<0.05 or P<0.001).Conclusion Effective nursing intervention can significantly increase the basic tear secretion,improve the function of tear film lipid layer,promote the repair of corneal and ocular surface damage and the recovery of meibomian gland secretion function,and then improve the clinical efficacy of MGD-induced evaporative DED.
