藤黄健骨胶囊联合碳酸钙d3咀嚼片及经皮椎体成形术治疗腰椎骨折合并严重骨质疏松患者的随访研究

Follow-up Study of Fujiigu Jiangu Capsule combined with Calcium Carbonate D3 Chewable Tablets and Percutaneous Vertebroplasty in the Treatment of Lumbar Fractures Complicated with Severe Osteoporosis

目的探讨藤黄健骨胶囊联合碳酸钙d3咀嚼片及经皮椎体成形术(PVP)治疗腰椎骨折合并严重骨质疏松(OP)患者的临床效果。方法选取2018年1月至2019年2月我院腰椎骨折合并严重OP患者84例,依据治疗方法不同分为研究组(n=42)、对照组(n=42)。两组均行PVP术,在此基础上对照组予以碳酸钙d3咀嚼片治疗,研究组予以藤黄健骨胶囊联合碳酸钙d3咀嚼片治疗,比较两组术前、术后视觉模拟评分法(VAS)评分、治疗前后血清骨钙素(BGP)、骨保护素(OPG)、骨密度水平及随访6个月骨质疏松性腰椎骨折发生率。结果术后3d、5d、7d研究组VAS评分低于对照组(P<0.05);治疗3个月后研究组骨密度、BGP、OPG水平高于对照组(P<0.05);随访6个月研究组骨质疏松性腰椎骨折发生率2.44%低于对照组15.00%(P<0.05)。结论藤黄健骨胶囊联合碳酸钙d3咀嚼片及PVP治疗腰椎骨折合并严重OP,能缓解患者术后疼痛,提高骨密度、血清BGP及OPG水平,降低骨质疏松性腰椎骨折发生率。

Objective To investigate the clinical effects of Tenghuang Jiangu Capsule combined with calcium carbonate d3 chewable tablets and percutaneous vertebroplasty(PVP)in the treatment of lumbar fractures complicated with severe osteoporosis(OP).Methods 84 patients with lumbar vertebrae fracture and severe OP from January 2018 to February 2019 in our hospital were enrolled.According to different treatment methods,they were divided into study group(n=42)and control group(n=42).PVP was performed in both groups.On the basis of this,the control group was treated with calcium carbonate d3 chewable tablets.The research group was treated with Tenghuangjianu capsule combined with calcium carbonate d3 chewable tablets.The preoperative and postoperative visual analogue scales were compared between the two groups.VAS scores,serum osteocalcin(BGP),osteoprotegerin(OPG),bone mineral density,and incidence of osteoporotic lumbar fractures at 6 months follow-up.Results The VAS scores of the study group were lower than those of the control group at 3d,5d and 7d after operation(P<0.05).The bone mineral density,BGP and OPG levels of the study group were higher than those of the control group after 3 months of treatment(P<0.05).The incidence of osteoporotic lumbar fractures in the study group was 2.44%lower than that in the control group(15.00%)(P<0.05).Conclusion Rattan Huangjian Capsule combined with calcium carbonate d3 chewable tablets and PVP for the treatment of lumbar vertebrae fracture combined with severe OP can alleviate postoperative pain,increase bone density,serum BGP and OPG levels,and reduce the incidence of osteoporotic lumbar fractures.
