白三烯受体拮抗剂联合糖皮质激素治疗嗜酸粒细胞性支气管炎患者的短期随访研究

Short-term Follow-up Study of Leukotriene Receptor Antagonists combined with Glucocorticoids in Patients with Eosinophilic Bronchitis

目的探讨白三烯受体拮抗剂联合糖皮质激素治疗嗜酸粒细胞性支气管炎(Eosinophilic bronchitis,EB)患者的短期随访效果。方法选取我院EB患者60例(2016年10月至2018年10月),依据随机数字表法分为对照组(n=30)、观察组(n=30)。常规治疗基础上,对照组采用糖皮质激素治疗,观察组在对照组基础上采用白三烯受体拮抗剂治疗。比较两组治疗前、治疗4周后咳嗽评分、Spitzer生活质量指数(SQOL)评分、血清白细胞介素-5(IL-5)水平、外周血嗜酸粒细胞(Eos)计数、呼出气一氧化氮(FeNO)浓度、随访半年复发率。结果与对照组比较,治疗4周后观察组咳嗽评分、血清IL-5水平、FeNO浓度较低,外周血Eos计数较少,SQOL评分较高(P<0.05);两组随访半年复发率比较,观察组20.69%(6/29)低于对照组48.28%(14/29,P<0.05)。结论白三烯受体拮抗剂联合糖皮质激素治疗EB患者,可缓解咳嗽,降低外周血Eos水平与复发率,减轻机体炎症反应,提高生活质量。

Objective To investigate the short-term follow-up effect of leukotriene receptor antagonist combined with glucocorticoid in patients with eosinophilic bronchitis(EB).Methods Sixty patients with EB in our hospital(October 2016 to October 2018)were enrolled and divided into control group(n=30)and observation group(n=30)according to the random number table method.On the basis of conventional treatment,the control group was treated with glucocorticoids,and the observation group was treated with leukotriene receptor antagonists on the basis of the control group.Comparison of cough score,Spitzer quality of life index(SQOL)score,serum interleukin-5(IL-5)level,peripheral blood eosinophil(Eos)count,exhaled breath monooxidation before treatment,4 weeks after treatment Nitrogen(FeNO)concentration,recurrence rate during the follow-up period of six months.Results Compared with the control group,after 4 weeks of treatment,the cough score,serum IL-5 level,FeNO concentration were lower in the observation group,the Eos count in the peripheral blood was less,and the SQOL score was higher(P<0.05).The observation group was 20.69%(6/29)lower than the control group 48.28%(14/29,P<0.05).Conclusion Leukotriene receptor antagonist combined with glucocorticoid in the treatment of EB patients can relieve cough,reduce the level of Eos and recurrence of peripheral blood,reduce the body’s inflammatory response and improve the quality of life.
