重症超声指导下的液体管理对ARDS患者的治疗效果

Therapeutic Effect of Fluid Management under the Guidance of Severe Ultrasound on ARDS Patients

目的探讨重症超声指导下的液体管理对ARDS患者的治疗效果。方法选取急性呼吸窘迫综合征患者53例,其中26例患者在治疗中接受的是CVP指标进行液体管理,为参照组;另外27例患者在治疗中接受的是重症超声指导下进行液体管理,为研究组;观察两组临床效果。结果参照组患者通气时间和住院时间显著高于研究组患者(P<0.05);参照组和研究组的心率、呼气末气道正压、血乳酸、急性生理和慢性健康评分均显著下降,氧合指数显著升高;但研究组患者的各项检测指标均显著优于参照组(P<0.05)。结论重症超声指导急性呼吸窘迫综合征患者的液体管理可减少通气时间和住院时间,检测患者的各项指标的安全性更高,在临床上属于一种容量评估的有效手段。

Objective To explore the therapeutic effect of fluid management under the guidance of severe ultrasound on ARDS patients.Methods Fifty-three patients with acute respiratory distress syndrome were selected,of which 26 patients received CVP index for fluid management in the treatment,as the reference group,27 patients received severe ultrasound guidance for fluid management in the treatment,as the study group.Observe the clinical effect of the two groups.Results The ventilation time and hospitalization time of the patients in the reference group were significantly longer than those in the study group(P<0.05).The heart rate,positive airway pressure at the end of expiratory period,blood lactate,acute physiological and chronic health scores in the reference group and the study group were significantly lower,and the oxygenation index was significantly higher;but the detection indexes of the patients in the study group were significantly better than those in the reference group(P<0.05).Conclusion The safety of each index of patients is higher,which is an effective means of capacity evaluation in clinic.
