上海市青浦区赵巷镇社区卫生服务中心作为上海市家庭医生中医药服务示范岗之一,在家庭医生服务团队中医全科医师签约服务模式研究探索中,探索出"简、便、验、廉"的中医药健康管理优化模式,提升团队综合服务水平。形成一套可复制可推广的中医药服务模式,起到示范引领作用,以期实现为社区居民提供方便、优效、全程及个性化的中医药指导服务。

The community health service center of zhaoxiang town,qingpu district,shanghai is one of the demonstration posts of traditional Chinese medicine service for family doctors in Shanghai,in the research and exploration of the service model of Chinese medicine general practitioners contracted by the family doctor service team,an"simple,convenient,efficient and inexpensive"Chinese medicine health management optimization model was explored to improve the team’s comprehensive service level.Form a set of replicable and popularized TCM service mode,play a leading role in demonstration,in order to provide convenient,efficient,full process and personalized TCM guidance services for community residents.
