对社区老年慢性病患者实施连续护理管理模式的效果观察

Observation on the Effect of Continuous Nursing Management Model for the Elderly Patients with Chronic Diseases in the Community

目的观察采取连续护理管理模式对社区老年慢性病患者干预后其治疗依从性、自我效能的积极影响价值。方法选取本社区100例老年慢性病患者,数字编序后通过盲目均等方式完成分组处理,各组有50例,对照组社区老年慢病患者采取常规治疗及护理干预,观察组社区老年慢病患者在常规治疗基础上配合连续管理干预模式。结果依从性观察组患者相关指标所得数据均显著优于对照组(P<0.05);自我效能,观察组患者提升幅度显著高于对照组(P<0.05)。结论为社区收治的老年慢病患者在家庭医生团队引导下进行连续护理管理模式能提高其对于自身疾病和生活方式有更加全面的认知,值得临床推广。

Objective To observe the positive effect of continuous nursing management mode on the treatment compliance and self-efficacy of elderly patients with chronic diseases after intervention.Methods One hundred elderly patients with chronic diseases in the community were selected.After digital sequencing,they were divided into groups by blind and equal way.There were 50 cases in each group.In the control group,the elderly patients with chronic diseases in the community took routine treatment and nursing intervention.In the observation group,the elderly patients with chronic diseases in the community cooperated with the continuous management intervention mode on the basis of routine treatment.Results The data of the compliance observation group was significantly better than that of the control group(P<0.05).The self-efficacy of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The continuous nursing management mode for the elderly patients with chronic diseases in the community under the guidance of the family doctor team can improve their comprehensive cognition of their own diseases and lifestyle,which is worthy of clinical promotion.
