吻合血管游离腓骨移植治疗股骨头缺血性坏死的临床研究

Clinical Study on Free Fibular Transplantation with Anastomotic Vessels in Treatment of Avascular Necrosis of Femoral Head

目的对股骨头缺血性坏死患者采用吻合血管游离腓骨移植治疗,并对其临床治疗效果进行研究。方法我院自2019年1月至2019年6月共收治单侧股骨头缺血性坏死患者70例,并采用Ficat分期标准根据其实际病情进行分期,并给予II、III期患者吻合血管游离腓骨移植治疗,记录术前疼痛评分、Harris髋关节功能评分、影像学评价,设为对照组。手术完毕后,进行随访,并将其末次随访时的Harris髋关节功能评分、影像学评价及时的疼痛评分进行记录,设为实验组,对比其疼痛评分、Harris髋关节功能评分。结果实验组组疼痛评分明显更低,且Harris髋关节功能评分均高于对照组(P<0.05)。结论吻合血管游离腓骨移植治疗临床意义显著,可作为II、III期股骨头缺血性坏死患者首选治疗措施。

Objective to study clinical curative effect of vascularized fibula transplantation in treatment of avascular necrosis of femoral head.Methods choose 70 cases unilateral avascular necrosis patients of femoral head in our hospital from January 2019 to June 2019.According to Ficat stage standard and actual condition,divide them into different stages,treated stage II and III with free fibula transplantation with anastomotic vessels.Record preoperative pain scores,Harris hip function scores and imaging evaluation as control group.After operation and follow-up,record Harris hip joint function scores,imaging evaluation and timely pain scores at the last follow-up as experimental group.Compare pain scores and Harris hip joint function scores.Results pain scores of experimental group were significantly lower,and Harris hip joint function scores were higher than control group(P<0.05).Conclusion free fibula transplantation with anastomotic vessels is of great clinical significance for treatment of stage II and III avascular necrosis of the femoral head,which can be the first choice.
