单纯甲状腺过氧化物酶抗体阳性对子代甲状腺功能的影响

Influence of Thyroid Peroxidase Antibody Levels of Pregnant Woman on Thyroid Function of Offspring

目的探讨孕妇单纯甲状腺过氧化物酶抗体(TPOAb)阳性对子代甲状腺功能的影响。方法选取2017年6月至2018年12月在我院妇产就诊建卡并符合入组标准的单纯TPOAb阳性孕妇20例作为TPOAb阳性组(子代为P组),另取同期产检健康孕妇19例作为阴性对照组(子代为N组)。比较各子代组的新生儿Apgar评分、身长和体重指标以及出生后脐血、1个月、3个月的甲状腺功能(TSH、FT4)指标以及TPOAb抗体水平;分析TPOAb阳性孕妇末次产检TPOAb水平与P组新生儿出生后脐血甲功指标和TPOAb水平的相关性。结果两组新生儿Apgar评分、身长和体重指标之间无显著性差异(P>0.05)。P组新生儿出生后TSH、TPOAb均显著高于N组,FT4则低于N组(P<0.05);P组新生儿出生后1月TSH、TPOAb均显著高于N组(P<0.05);出生后3月,两组新生儿TSH、TPOAb、FT4指标无显著性差异(P>0.05)。P组新生儿出生后TSH、TPOAb、FT4与母体TPOAb水平强相关(P<0.05)。结论孕妇单纯TPOAb阳性可能对子代出生后甲状腺功能产生一定影响。

Objective To investigate the influence of Thyroid Peroxidase Antibody levels of pregnant woman on thyroid function of offspring.Methods 20 TPOAb positive alone pregnant women,admitted to our hospital between June.2017~December.2018,who met the entry criteria were selected as TPOAb positive group,whose neonates were assigned into P group.19 Health pregnant women during same period were selected as TPOAb negative group,whose neonates were assigned into N group.Compare the Apgar score,length,weight,TSH,FT4,TPOAb(after birth,1 month after birth,3 months after birth)between P and N group.Analyze the correlation between TPOAb levels in pregnant women and TSH,FT4,TPOAb levels in P group.Results The Apgar score,length,weight between P and N groups had no significant difference(P>0.05).After birth,The TSH and TPOAb levels of P group were much higher than N group,and FT4 was much lower than latter(P<0.05).1 month after birth,The TSH and TPOAb levels of P group were much higher than N group(P<0.05).3 months after birth,All the indicates had no significant difference between P and N group.Strong correlation was existed between the TSH,FT4,TPOAb levels after birth in P group and TPOAb level in maternal.Conclusion TPOAb positive alone pregnant women may have some effect on the thyroid function of their offspring after birth.
