老年脑外伤扩大去骨瓣减压术后硬膜下积液合并脑积水的危险因素

Risk Factors of Subdural Effusion Combined with Hydrocephalus after Extended Decompression of Craniectomy in Elderly Patients with Brain Trauma

目的对老年脑外伤扩大去骨瓣减压术后硬膜下积液合并脑积水的危险因素进行分析。方法选取我院2013年至2018年收治的382例老年脑外伤扩大去骨瓣减压术患者作为此次研究的对象,分析患者的相关基础资料,研究各种因素与硬膜下积液合并脑积水发生的关系,单因素采用χ~2进行分析,多因素采用Logitic回归进行分析。结果通过相关分析得知,在382例患者中有130例术后出现了硬膜下积液合并脑积水症状,此并发症与脑内室出血、硬膜下积液诊断时长以及纵裂间积液具有密不可分的联系。结论掌握硬膜下积液合并脑积水的相关危险因素,针对各类因素采取针对性的预防措施,可有效减少老年脑外伤扩大去骨瓣减压术后出现的硬膜下积液合并脑积水的发生几率,同时避免了老年人残疾和死亡的风险。

Objective To analyze the risk factors of subdural effusion combined with hydrocephalus after decompressive craniectomy in elderly patients with traumatic brain injury.Methods Three hundred and eighty-two elderly patients with brain trauma undergoing extended decompressive craniectomy from 2013 to 2018 were selected as the subjects of this study.The basic data of the patients were analyzed.The relationship between various factors and the occurrence of subdural effusion combined with hydrocephalus was studied.χ~2 were used for single factor analysis and Logitic regression was used for multiple factors analysis.Results According to relevant analysis,130 of 382 patients had postoperative symptoms of subdural effusion and hydrocephalus.This complication was associated with intraventricular hemorrhage,diagnosis of subdural effusion,and longitudinal fissure.Interstitial fluids are inextricably linked.Conclusion To grasp the risk factors of subdural effusion combined with hydrocephalus and take appropriate preventive measures against various factors can effectively reduce the incidence of subdural effusion combined with hydrocephalus after extended decompression of craniectomy in elderly patients with traumatic brain injury,and avoid the risk of disability and death in the elderly.
