98例原发性IgA肾病患者临床特点与病理分析

Clinical Characteristics and Pathological Analysis of 98 Cases Primary IgA Nephropathy Patients

目的探究原发性IgA肾病患者的临床特点与病理分级之间的关系。方法选取我院收治的98例原发性IgA肾病患者做回顾性分析,分析并总结患者的临床资料,分析患者临床特点及病理分级。结果据研究结果显示,原发性IgA肾病患者的病理分级越高,BUN指标随之上升,eGFR随之下降,差异具有统计学意义(P<0.05),不同病理分级与年龄、血压之间无统计学意义(P>0.05)。原发性IgA肾病患者临床表现为,无症状尿检异常性,达45.92%,其次为肾病综合征,达35.71%,再次为高血压型,达9.18%,终末IgA期型为2.04%,所占比例最低。结论原发性IgA肾病主要多发于青壮年,临床表现较为多样,主要以无症状尿检异常为主,且病理分级越高,患者预后效果越差,故早期进行肾活检及分析病理分级对判断患者预后具有重要的临床价值与意义。

Objective to explore relationship between clinical characteristics and pathological grade of primary IgA nephropathy patients.Methods choose 98 cases primary IgA nephropathy patients in our hospital for retrospective analysis,analyze clinical data,clinical characteristics and pathological grading.Results research results showed,the higher pathological grade of primary IgA nephropathy patients,the higher bun index,the lower EGFR.Difference was statistically significant(P<0.05).There was no statistical significance between different pathological grades and age,blood pressure(P>0.05).Clinical manifestations of primary IgA nephropathy patients were asymptomatic abnormal urinalysis(45.92%),followed by nephrotic syndrome(35.71%),hypertension(9.18%)and terminal IgA(2.04%),with the lowest proportion.Conclusion primary IgA nephropathy mainly occurs in young adults,with various clinical manifestations,mainly asymptomatic abnormal urinalysis,and the higher pathological grade,the worse prognosis of patients,early renal biopsy and pathological grade analysis has important clinical value and significance in diagnosing prognosis of patients.
