卵巢囊肿采取经脐单孔腹腔镜术与传统腹腔镜术治疗对比分析

Comparative Analysis of Ovarian Cysts Treated with Transumbilical Singleport Laparoscopic Surgery and Conventional Laparoscopic Surgery

目的探讨经脐单孔腹腔镜和传统腹腔镜行卵巢囊肿剔除术的临床效果。方法2018年3月至2018年12月期间收治的卵巢囊肿患者80人随机分为两组,观察组40人,为经脐单孔腹腔镜卵巢囊肿剔除术;对照组40人,为传统腹腔镜卵巢囊肿剔除术。收集两组患者手术指标、术前、术后卵巢功能改变情况,观察比较。结果与对照组相比,观察组手术时间更长,但首次排气时间、住院时间更短,术中出血量更多,VAS评分更低,差异具有统计学意义(P<0.05);术前、术后两组患者雌二醇(E2)、促黄体生成素(LH)、促卵细胞激素(FSH)各项卵巢功能指标均无较大差异(P>0.05)。结论经脐单孔腹腔镜和传统腹腔镜行卵巢囊肿剔除术治疗卵巢囊肿均可一定程度上改善患者卵巢功能,但经脐单孔腹腔镜卵巢囊肿剔除术创伤更小,腹部切口美观,脐部切口经使用罗哌卡因局部麻醉后疼痛比传统腹腔镜术后腹部切口疼痛轻,术后患者恢复更快,住院时间短,满意度高。

Objective To investigate the clinical effects of laparoscopic single-hole laparoscopy and conventional laparoscopic ovarian cyst removal.Methods Eighty patients with ovarian cysts from March 20 to December 2018 were randomly divided into two groups.The observation group consisted of 40 patients with laparoscopic laparoscopic ovarian cyst removal.The control group consisted of 40 patients with conventional abdominal cavity.Mirror ovarian cyst removal.The surgical indexes,preoperative and postoperative ovarian function changes of the two groups were collected and compared.Results Compared with the control group,the observation group had longer operation time,but the first exhaust time and hospitalization time were shorter,the intraoperative blood loss was more,and the VAS score was lower,the difference was statistically significant(P<0.05).There were no significant differences in ovarian function indexes between estrogen(E2),luteinizing hormone(LH)and stimulating cytokines(FSH)between the two groups(P>0.05).Conclusion Transumbilical single-port laparoscopy and conventional laparoscopic ovarian cyst removal for ovarian cysts can improve the ovarian function to some extent,but the umbilical single-port laparoscopic ovarian cyst removal surgery has less trauma,beautiful abdominal incision,umbilical cord.The pain in the incision after local anesthesia with ropivacaine was lighter than that in the conventional abdominal laparoscopic abdominal incision.The postoperative patient recovered faster,the hospital stay was short,and the satisfaction was high.
