新型冠状病毒肺炎患者创伤后应激障碍发生情况调查分析

Investigation of the impact of event in patients with coronavirus disease 2019 and its analysis of the related factors

目的了解新型冠状病毒肺炎患者创伤后应激障碍现状,并分析其相关因素。方法选取武汉市肺科医院收治确诊的65例患者作为研究对象。收集患者临床资料,采用事件影响量表评估患者发生创伤后应激障碍状况;采用社会支持评定量表、自尊量表和医院焦虑抑郁量表调查社会支持和心理状况。结果新型冠状病毒肺炎患者事件影响总分为(28.71±18.34)分。患者社会支持评分、自尊评分焦虑抑郁评分分别为(43.56±7.40)分、(30.90±3.46)分和(10.18±3.15)分。不同年龄、基础疾病患者事件影响得分差异有统计学意义(P<0.05),患者社会支持与事件影响得分负相关(P<0.05),而焦虑抑郁与事件影响得分正相关(P<0.05)。结论高龄、患有基础疾病、社会支持、焦虑抑郁与新型冠状病毒型肺炎患者发生创伤后应激障碍有关。

Objective To investigate the status of post-traumatic stress disorder(PTSD) in patients with 2019 coronavirus disease 2019(COVID-19) and analyze the influencing factors. Methods 65 patients admitted to Wuhan pulmonary hospital(the first batch of COVID-19 treatment hospitals) were selected as study subjects. The clinical data of the patients were collected. The impact of event scale(IES-R) was used to evaluate the status of post-traumatic stress disorder. The social support rating scale(SSRS),self-esteem scale(SES) and hospital anxiety and depression scale(HADS) were also used to evaluate social support and psychological status. Single factor analysis was employed to analyze the influenced factors for PTSD. Results IES-R scores of invasion,avoidance and high arousal of patients with COVID-19 were(10.85±6.66),(10.40±7.06) and(7.46±5.55),and the total score of IES-R was(28.71±18.34). SSRS scores,SES scores and HADS scores were(43.56±7.40),(30.90±3.46)and(10.18±3.15),respectively. Single factor analysis showed PTSD symptom scores among patients with different ages,underlying diseases,SSRS scores,SES scores and HADS scores presented significant differences(P <0.05). Pearson analysis confirmed the social support was negative related to the PTSD symptom scores,while anxiety and depression was positive related to the PTSD symptom scores(P<0.05). Conclusion Higher age,underlying basical disease,low social support and self-esteem,and serious HADS were related with PTSD symptoms.
