新型冠状病毒肺炎疫情期间对乳腺癌患者的个案管理

Case Management of Breast cancer patients during the coronavirus disease 2019 outbreak

目的探讨新型冠状病毒肺炎疫情期间乳腺癌患者的个案管理经验,旨在为疫情期间对乳腺癌患者的全程管理提供参考。方法疫情期间,广州市某三级医院乳腺科个案管理师通过联合多学科体系,在乳腺癌患者的确诊期、围手手术期、辅助治疗期、康复期实施个案管理。结果 2020年2月3日—3月6日,乳腺科门诊接诊患者3 080例,入院收治患者102例,尚未发现疑似病例。疫情发生前个案管理师所管理的1 545例患者中,在疫情期间辅助治疗率达81.3%~92.1%,复诊率达87.9%。结论疫情期间,通过个案管理师的全程管理,帮助乳腺癌患者减少外出频率,降低交叉感染风险,有效解决就医难题,确保了个案治疗的有序性和规范性,提高了临床护理质量,减轻患者心理压力,为本次及未来突发公众事件下乳腺癌患者的全程管理提供参考。

Objective To explore the case management experience of breast cancer patients during the outbreak of Corona Virus Disease 2019,so as to provide references for the whole-course management of breast cancer patients during the outbreak. Methods During the outbreak,the case manager in the Department of Breast,Sun YatSen University Cancer Center,carried out case management in the diagnosis,perioperative,adjuvant treatment and rehabilitation stages of breast cancer patients by combining multidisciplinary systems. Results From February 3 to March 6,2020,the hospital’s breast department has received 3080 outpatient patients and 102 admitted patients,no suspected cases were found. Among the 1545 patients under the care of the case manager before the outbreak,the rate of adjuvant treatment during the epidemic period was 81.3% ~92.1%,and the rate of follow-up was 87.9%.Conclusion During the outbreak,case management reduced the frequency of patients going out and the risk of cross-infection,effectively solved the problem of seeking a doctor,ensured the orderliness and standardization of case treatment,improved the quality of clinical care,reduced the psychological pressure of patients. This study provides references for the whole-process management of breast cancer patients in this and future public emergencies.
