3H护理模式对胃癌患者术后应激及免疫功能的影响

Effects of “3H ” nursing mode on postoperative stress and immune function in patients with gastric cancer

目的探讨"3H"护理模式对胃癌患者术后应激及免疫功能的影响。方法选取2017年7月至2018年7月间辽宁省抚顺市中医院收治的164例胃癌患者作为研究对象。根据采用的护理方法不同进行分组,其中,采用常规护理的82例患纳入对照组,采用"3H"模式护理的82例患者纳入观察组。比较两组患者干预前后的应激反应和免疫功能指标,并分析并发症发生率。结果两组患者干预后白细胞介素-6(IL-6)、白细胞介素-10(IL-10)和肿瘤坏死因子-α(TNF-α)水平均明显升高,CD4+/CD8+和自然杀伤细胞(NK)均明显下降,差异均有统计学意义(均P<0.05)。观察组干预后IL-6、IL-10和TNF-α水平均低于对照组,且CD4+/CD8+和NK均免疫功能指标均高于对照组,差异均有统计学意义(均P<0.05)。观察组消化道症状、消化道出血、切口感染和深静脉血栓发生率均显著低于对照组,差异均有统计学意义(均P<0.05)。结论"3H"护理模式可显著降低胃癌患者术后应激反应,减轻患者免疫功能抑制程度,并降低并发症发生率。

Objective To explore the effect of"3 H"nursing mode on postoperative stress and immune function in patients with gastric cancer.Methods A total of 164 patients with gastric cancer who admitted to Fushun Hospital of Traditional Chinese Medicine from July 2017 to July 2018 were selected as the research subjects.Of them,82 patients who received"3 H"nursing mode-based nursing care were included in an observation group and 82 patients who received conventional nursing care were included in a control group.Stress and immune function indexes were compared between the two groups before and after the intervention.The incidence of complications was analyzed.Results After intervention,interleukin-6(IL-6),IL-10 and tumor necrosis factor-α(TNF-α)levels increased and CD4+/CD8+and natural killer cell(NK)levels decreased in both groups(all P<0.05).The levels of IL-6,IL-10 and TNF-αwere lower in the observation group than in the control group and CD4+/CD8+and immune function indexes were higher in the observation group than in the control group(all P<0.05).The incidence of gastrointestinal symptoms,gastrointestinal bleeding,incision infection and deep vein thrombosis was significantly lower in the observation group than in the control group(all P<0.05).Conclusion"3 H"nursing mode-based care can significantly reduce postoperative stress response,weaken the degree of immunosuppression,and reduce the incidence of complications in patients with gastric cancer.
