针对性护理对乳腺癌根治术后化疗患者不良反应与生活质量的影响 被引量:34

Effect of targeted nursing on adverse reactions and quality of life in patients undergoing chemotherapy after radical mastectomy

摘要 目的探讨针对性护理对乳腺癌根治术后化疗患者不良反应与生活质量的影响。方法选取2016年1月至2018年12月间陕西省渭南市中心医院收治的乳腺癌根治术后实施化疗治疗的182例女性患者,根据护理方法不同分组,其中,采用常规护理干预措施的91例患者纳入对照组,在常规护理基础上采用针对性护理措施的91例患者纳入研究组。比较两组患者化疗期间出现的药物不良反应和干预前后的生活质量。结果研究患者组术后化疗期间胃肠道反应、静脉炎和口腔炎的发生率均低于对照组,差异均有统计学意义(均P<0.05)。两组患者骨髓抑制发生率比较,差异无统计学意义(P>0.05)。护理干预后,研究组患者社会关系、生理、心理、环境、独立性和精神评分均高于对照组患者,差异均有统计学意义(均P<0.05)。结论针对性护理能够有效改善乳腺癌根治术后化疗患者的不良反应,提高生活质量,值得临床推广。 Objective To explore the effect of targeted nursing on adverse reactions and quality of life in patients undergoing chemotherapy after radical mastectomy.Methods From January 2016 to December 2018,182 women with breast cancer who underwent chemotherapy after radical mastectomy were selected as the study subjects.They were divided into groups by random number table method,of which 91 patients were included in the study group and 91 patients were included in the control group.The study group was given targeted nursing interventions besides routine nursing interventions while the control group was given routine nursing interventions during chemotherapy.The adverse drug reactions during chemotherapy were compared between the two groups.Quality of life scale was used to evaluate the quality of life in the two groups before and after nursing intervention.Results The incidence of gastrointestinal reactions,phlebitis and stomatitis were lower in the study group than in the control group(all P<0.05).There was no significant difference in the incidence of bone marrow suppression between the two groups(P>0.05).Before the intervention,there was no significant difference in the scores of social relationship,physiology,psychology,environment,independence and spirit between the two groups(P>0.05).After the intervention,the scores of social relationship,physiology,psychology,environment,independence and spirit were higher in the study group than in the control group respectively(all P<0.05).Conclusion Targeted nursing can effectively improve the adverse reactions and quality of life in patients undergoing chemotherapy after radical mastectomy,which is worthy of clinical promotion.
