重庆地区HIV感染者和AIDS患者合并恶性肿瘤疾病谱分析

Disease spectrum analysis in HIV/AIDS patients with malignant tumor in Chongqing

目的了解重庆地区人类免疫缺陷病毒(human immunodeficiency virus,HIV)感染者和获得性免疫缺陷综合征(acquired immunodeficiency syndrome,AIDS)患者(HIV感染者/AIDS患者)合并恶性肿瘤住院者的疾病谱特征和变化趋势。方法回顾性分析2016年1月至2020年12月重庆市公共卫生医疗救治中心感染科收治的肿瘤住院患者的临床资料。结果2016—2020年HIV感染者/AIDS患者合并恶性肿瘤患病率依次为1.85%(29/1570)、2.88%(49/1702)、2.86%(55/1922)、4.45%(122/2742)、4.94%(113/2286),总体呈逐年上升趋势,差异有统计学意义(χ^(2)=37.051,P<0.01)。前五位肿瘤分别为淋巴瘤126例、肺癌94例、卡波西肉瘤39例、肝癌20例、颅内恶性肿瘤13例。男性HIV感染者/AIDS患者常见肿瘤为淋巴瘤32.35%(99/306)、肺癌27.78%(85/306)、卡波西肉瘤12.42%(38/306),女性分别为淋巴瘤43.55%(27/62)、肺癌14.52%(9/62)、宫颈癌12.90%(8/62)。老年患者以肺癌最多见,占38.29%(85/222),非老年患者以淋巴瘤常见,占50.68%(74/146)。结论重庆地区HIV感染者/AIDS患者合并恶性肿瘤住院患者呈现逐年增长的趋势,存在性别、年龄分布差异,病种以淋巴瘤、肺癌常见,同时应该重视卡波西肉瘤、肝癌、颅内恶性肿瘤、宫颈癌等恶性肿瘤的防治。

Objective To understand the characteristics and trends of disease spectrum of HIV/AIDS inpatients with malignant tumor in Chongqing.Methods The clinical data of patients with tumor admitted to the Infection Department of Chongqing Public Health Medical Treatment Center from January 2016 to December 2020 were retrospectively analyzed.Results The annual incidence of malignant tumors in HIV/AIDS cases was 1.85%(29/1570)in 2016,2.88%(49/1702)in 2017,2.86%(55/1922)in 2018,4.44%(122/2742)in 2019 and 4.94%(113/2286)in 2020,which showed an up-ward trend and the differences were significant(χ^(2)=37.051,P<0.01).The top five malignant tumors were lymphoma(n=126),lung cancer(n=94),Kaposi′s sarcoma(n=39),liver cancer(n=20)and intracranial malignancy(n=13).In male HIV/AIDS patients,the common malignant tumors were lymphoma(99/306),lung cancer(85/306)and Kaposi′s sarcoma(38/306),while in female HIV/AIDS cases,they were lymphoma(27/62),lung cancer(9/62)and cervical cancer(8/62).Lung cancer was the most common tumor in elder HIV/AIDS patients,accounting for 38.29%(85/222),while in younger HIV/AIDS patients,lymphoma was the most common one,accounting for 50.68%(74/146).Conclusions The incidence of malignant tumor in hospitalized HIV/AIDS patients.
