稳定期哮喘患者肺功能与哮喘控制测试量表与生活质量的关系研究

Relationship between lung function, asthma control test score and quality of life in patients with stable asthma

目的探讨哮喘稳定期患者肺功能与哮喘控制测试量表(ACT)与提高生活质量的关系。方法选取2017年6月~2018年6月海南省中医院呼吸内科收治的86例哮喘稳定期的患者进行肺功能检查,记录患者DOSE指数及生活质量,采用Pearson单因素分析哮喘稳定期患者肺功能与ACT评分及生活质量的关系。结果肺功能为Ⅰ级~Ⅱ级的哮喘患者肺活量(FVC)为(1.98±0.22)L、(1.72±0.42)L,第1秒用力呼气容积/用力肺活量(FEV1/FVC)为(71.22±4.11)%、(62.38±3.85)%,哮喘控制测试量表(ACT)得分为(23.12±4.85)分、(20.25±3.52)分,明显高于Ⅲ级~Ⅳ级的(1.45±0.51)L、(1.22±0.39)L、(51.36±4.02)%、(47.22±3.96)%、(17.11±3.82)分、(12.89±3.10)分;肺功能为Ⅰ级~Ⅱ级的哮喘患者6 min步行距离(6MWT)为(0.38±0.11)分、(1.02±0.45)分,明显低于Ⅲ级~Ⅳ级的(1.75±0.68)分、(2.52±0.89)分,差异均有统计学意义(P<0.05)。哮喘稳定期患者ACT评分与FEV1%、FEV1/FVC、6MWT及生活质量总评分呈正相关(P<0.05)。结论哮喘稳定期患者肺功能与ACT生活质量有密切关系,ACT及生活质量可用于患者病情自我监测。

Objective To describe the relationship between lung function,asthma control test(ACT)score and quality of life in patients with stable asthma.Methods 86 patients with stable asthma treated in our hospital from June 2017 to June 2018 were enrolled.Pulmonary function test and ACT were performed and the quality of life was assessed.Pearson single factor analysis model was used to analyze the relationship between lung function,ACT score and quality of life of patients.Results The vital capacity(FVC),the forced expiratory volume/forced vital capacity(FEV1/FVC)ratios and ACT scores were significantly higher in asthmatic patients with pulmonary function of grade I and II than those with pulmonary function of grade III and IV(1.98±0.22)L and(1.72±0.42)L vs(1.45±0.51)L and(1.22±0.39)L,(71.22±4.11)%and(62.38±3.85)%vs(51.36±4.02)%and(47.22±3.96)%,(23.12±4.85)and(20.25±3.52)vs(17.11±3.82)and(12.89±3.10),while the score of 6 M walking test(6 MWT)was significantly lower(0.38±0.11 and 1.02±0.45 vs 1.75±0.68 and 2.52±0.89),the differences were statistically significant(P<0.05).Pearson single factor analysis showed that the ACT score,FEV1/FVC ratio,6 MWT score in patients with stable asthma were positively correlated with the quality of life(P<0.05).Conclusions In patients with stable asthma,lung function and ACT score are closely related with quality of life,which can be used vy patients for self-monitoring of their own diseases.
