幽门螺旋杆菌感染对冠心病患者血脂及肌酐清除率影响的研究

The association of Helicobacter pylori infection with serum lipidlevel and creatinine clearance in patients with coronary heart disease

目的探索幽门螺杆菌(helicobacter pylori,Hp)感染对冠心病患者肌酐清除率的影响。方法选取2015年12月至2017年12月于绍兴市人民医院接受治疗的596例冠心病患者为研究对象,按照是否发生Hp感染将患者分为感染组(n=254)和未感染组(n=342)。采集患者血清样本并检测肌酐水平,计算患者肌酐清除率,检测患者Hp感染情况;检测患者甘油三酯(triylgceride,TG)、高密度脂蛋白胆固醇(high density liptein cholest erol,HDL-C)、低密度脂蛋白胆固醇(low density liptein cholest erol,LDL-C)、总胆固醇(totalcholesterd,TC)指标水平。结果Hp感染组和未感染组患者性别、年龄对比差异无统计学意义(P>0.05),感染组空腹血糖水平为(7.98±1.32)mmHg高于未感染组空腹血糖水平(3.28±1.17)mmHg,数据对比差异有统计学意义(t=2.289,P<0.05)。感染组和未感染组患者LDL-C、TC指标水平对比差异无统计学意义(P>0.05),感染组TG指标水平为(2.49±1.25)mmol/L高于未感染组(1.30±0.61)mmol/L,HDL-C为(1.13±0.26)mmol/L低于未感染组(1.98±0.57)mmol/L,数据对比差异有统计学意义(t=2.389、2.091,P均<0.05)。冠心病患者Hp感染与肌酐清除率降低存在显著相关性(P<0.05),同时冠心病患者动脉内径狭窄支数分组和冠心病病程分组中,Hp感染患者发生肌酐清除率降低可能性高于未感染患者(P<0.05)。结论Hp感染会影响患者血脂指标水平,Hp感染与冠心病患者肌酐清除率降低存在密切关系。

Objective To explore the association of Helicobacter pylori(Hp)infection with serum lipid level and creatinine clearance in patients with coronary heart disease(CHD).Methods 596cases with coronary heart disease treated in our hospital from December 2015to December 2017were enrolled into the study and were divided into Hp infected group(n=254)and uninfected group(n=342).Blood samples were collected and serum creatinine level was detected,the creatinine clearance rate of patients was calculated.Meanwhile,the serum levels of triglyceride(TG),high density lipoprotein(HDL-C),low density lipoprotein(LDL-C)and total cholesterol(TC)were also determined.Results There were no significant differences in gender and age between the two groups(P>0.05).The serum levels of fasting blood glucose(7.98±1.32)mmHg and TG(2.49±1.25)mmol/L of the infected group were,significantly higher than that of the uninfected group(3.28±1.17)mmHg;(1.30±0.61)mmol/L(P<0.05),while the HDL-C level was significantly lower(1.13±0.26 vs 1.98±0.57)mmol/L(P<0.05).There were no statistical differences in serum levels of LDL-C and TC between the two groups(P>0.05).Hp infection was closely correlated with the creatinine clearance rate in patients(t=2.389,2.091,P<0.05).The decrease of creatinine clearance rate in Hp infected patients with CHD was significantly higher than that in uninfected patients(P<0.05).Conclusions Hp infection in patients with CHD may affect the serum lipid level and also closely related with the reduced creatinine clearance rate.
