清心运脾法治疗小儿麦粒肿临证体会

Clinical experience of Qingxin Yunpi method in treating pediatric hordeolum

麦粒肿是儿科临床中常见的眼科疾病,常反复发作,给患儿带来较大的不适与痛苦,患儿因不适常用手揉搓,存在继发感染的风险。针对麦粒肿的治疗,目前西医尚无特效药物及方法,主要是眼部应用抗生素滴眼液或软膏、口服抗生素,已成脓肿等病情严重者需行外科手术切开排脓治疗。由于患儿心理恐惧、依从性较差等原因,外科手术治疗存在一定的局限性。中医药治疗小儿反复发作而尚未达到手术指征的麦粒肿具有较大的优势,内服中药疗效确切。冯晓纯教授认为小儿麦粒肿发病多因"热毒",与脾、心、肝等脏有关,临床治疗以清心运脾为大法,内服中药配合小儿推拿、刮痧、穴位贴敷等中医外治法临床疗效显著。此外,为避免疾病反复发作,还要注重儿童的生活护理,改善不良的饮食、生活习惯。

Hordeolum is a common and recurrent ophthalmic disease in pediatric clinical practice, which brings great discomfort and pain to children. Because of discomfort, children often rub their eyes with hands so there is a risk of secondary infection. Currently, there is no specific medicine and method for the treatment of hordeolum in western medicine. The main treatment methods are antibiotic eye drops or ointment, or oral antibiotics. Patients with severe conditions such as abscesses must undergo surgical incision and drainage. Because of the psychological fear and poor compliance, surgical treatment has certain limitations. Traditional Chinese medicine(TCM) has a great advantage in the treatment of recurrent children cases which are not the indication of operation. Professor Feng Xiaochun believes that the occurrence of hordeolum is mostly due to heat toxin, which is related to spleen, heart and liver. The method of clearing the heart and transporting the spleen is the major method for clinical treatment, and decoction following above method combined with massage, scrapping and acupoint application has a significant clinical effect. In addition, in order to avoid recurrence of the disease, we must pay attention to life care of children and improvement of poor diet and living habits.
