“骨搬移哈尔滨现象”组织转化再生原理的发现与临床意义 被引量:3

摘要 Ilizarov医师发明了环形外固定器、骨延长与骨搬移再生成骨技术,发现了牵拉组织再生原理(DH),即各种组织(包括骨骼、神经、血管、肌肉、皮肤等)在缓慢定量的牵拉作用下,均表现出极强的再生能力。在运用骨搬移技术治疗骨缺损的过程中发现,瘢痕组织在缓慢有节律的牵拉过程中按照形态和功能需求可转化成正常组织,并命名为"骨搬移哈尔滨现象",即在缓慢有节律的拉应力和压应力作用下,缺损间隙内会发生瘢痕等病理组织的转化再生现象。临床上可以不切除骨缺损内瘢痕组织、不进行植骨,运用自身转化再生能力治愈骨缺损。该现象与DH互为补充,共同成为指导临床骨搬移治疗的基础。 Dr.Ilizarov invented the Ilizarov external fixator,developed the bone lengthening and bone transport techniques,and discovered the principle of distraction histogenesis(DH),that is,various tissues(including bones,nerves,blood vessels,muscles,skin,etc.)show strong regenerative ability under the slow and rhythmic distraction.In the process of using Ilizarov’s bone transport technique to treat bone defects,we discovered that the scar tissue could be transformed into normal tissues according to the morphological and functional requirements under the slow and rhythmic distraction/compression,and named it as the"Harbin phenomenon of bone transport".Abnormal tissues in the bone defect area show strong transformation and regeneration abilities under the slow and rhythmic distraction/compression.This principle has proved that it is possible to heal the bone defect based on its own transformation and regeneration abilities without removing the scar tissue in the bone defect and without grafting bone.The"transformation histogenesis"and DH principles complement each other,and together become the basis for guiding clinical bone transport treatment.
