上海市儿童肠道病毒71型灭活疫苗接种率和疑似预防接种异常反应监测

Surveillance for coverage of inactivated enterovirus 71(EV71) vaccine and adverse events following immunization with EV71 vaccine among children in Shanghai

目的了解上海市肠道病毒71型(EV71)灭活疫苗接种率和疑似预防接种异常反应(AEFI)发生率。方法通过上海市免疫规划信息系统、中国AEFI监测信息管理系统收集2013年10月-2019年3月出生儿童EV71疫苗接种和AEFI数据,分析EV71疫苗接种率和AEFI发生率。结果上海市2013年10月-2019年3月出生儿童EV71疫苗1剂次接种率为38.30%,2剂次(全程)接种率为35.05%。本市、外来人口1剂次接种率分别为46.34%、33.89%,2剂次接种率分别为43.61%、30.36%;各年龄组1剂次接种率在20.49%-52.71%之间,2剂次接种率在17.34%-49.85%之间。EV71疫苗接种后AEFI报告发生率为224.91/10万剂(3046例);其中一般反应、异常反应分别为217.08/10万剂(2940例)、6.87/10万剂(93例),分别以发热(2263例,167.09/10万剂)、过敏性皮疹(87例,6.42/10万剂)为主。结论上海市儿童EV71疫苗接种率不高,AEFI报告发生率在预期范围内;仍应加强EV71疫苗接种宣传,提高儿童接种率。

Objective To analyze coverage of inactivated enterovirus 71(EV71)vaccine and incidence of adverse events following immunization(AEFIs)with EV71 vaccine among children in Shanghai.Methods We collected data on EV71 vaccination and AEFI cases among children born from October 2013 to March 2019 and reported to the Shanghai Immunization Information Management System and the China AEFI Information Management System;we assessed EV71 vaccine coverage and AEFI incidence.Results Coverage rates for one dose and two doses of EV71 vaccine were 38.30%and 35.05%,respectively,among children born from October 2013 to March 2019.Coverage rates for one dose of EV71 vaccine were 46.34%and 33.89%among local and migrant children,and coverage levels for two doses were 43.61%and 30.36%among local and migrant children,respectively.Coverage rates by age for one dose of EV71 vaccine ranged from 20.49%to 52.71%;coverage for two doses ranged f rom 17.34%to 49.85%.The reported EV71 vaccine AEFI incidence was 224.91 cases per 100000 doses(3046 cases total)with 217.08 per 100000(2940 cases)for common adverse reactions and 6.87 per 100000(93 cases)for rare adverse reactions;most common adverse reactions were fever(2263 cases,167.09 per 100000)and most rare adverse reactions were allergic rash(87 cases,6.42 per 100000).Conclusions Coverage of EV71 vaccination was not high among children in Shanghai,and the AEFI incidence of EV71 vaccine was not higher than the expected.We should strengthen health education to improve use of EV71 vaccine among children.
