日常生活能力、社会经济地位与老年人认知功能的关系

Relationship among daily living ability,socioeconomic status and cognitive function of the elderly

目的探讨日常生活能力、社会经济地位与老年人认知功能的关系,为控制及预防老人认知功能障碍提供依据。方法基于2018年中国老年健康影响因素跟踪调查(CLHLS)数据筛选获得10787名老年人,采用多因素Logistic回归模型分析老年人认知功能受损的影响因素及与日常生活能力和社会经济地位的关联。结果本研究中老人认知功能受损比例为24.0%。日常生活能力受损(57.7%)和低社会经济地位(42.8%)的老人认知功能受损比例较高(P均<0.001)。在调整混杂因素后,日常生活能力和社会经济地位的交互作用与认知功能存在相关性,以日常生活能力未受损且高社会经济地位组为参照,日常生活能力受损且低社会经济地位老人的认知功能受损风险最高(OR=6.184,95%CI:4.790~7.983)。结论日常生活能力受损会联合社会经济地位降低增加老人的认知功能受损风险,要综合考虑两者给老人认知功能带来的不利影响。

Objective To explore the relationship among daily living ability,social and economic status and cognitive function in the elderly,and to provide a basis for the control and prevention of cognitive dysfunction in the elderly.Methods Based on the 2018 China Elderly Health Impact Factors tracking Survey(CLHLS)data screening,10,787 elderly people were obtained.Multivariate logistic regression model was used to analyze the influencing factors of impaired cognitive function and their association with daily living ability and socioeconomic status.Results The proportion of impaired cognitive function in this study was 24.0%.The elderly people with impaired daily living ability(57.7%)and low socioeconomic status(42.8%)had higher proportion of impaired cognitive function(all P<0.001).After adjusting for confounders,the elderly people with impaired daily living ability and low socioeconomic status had the highest risk of impaired cognitive function(OR=6.184,95%CI:4.790-7.983)compared to the group with unimpaired daily living ability and high socioeconomic status.Conclusion Impaired daily living ability and reduced socioeconomic status will jointly increase the risk of impaired cognitive function of the elderly,attention should be paid attention to different treatment and prevention.
