健康教育对PICC置管肿瘤患者心理弹性、自我效能感及导管相关并发症的影响 被引量:4

Effect of health education on psychological resilience,self-efficacy and catheter-related complications in PICCinserted oncology patients

摘要 目的探讨基于某宣教平台的健康教育对PICC置管肿瘤患者心理弹性、自我效能感及导管相关并发症的影响,为临床改进健康宣教策略提供依据。方法选取2021年1—12月锦州医科大学附属第一医院PICC置管肿瘤患者287例,采用随机数字表法分组,对照组的143例患者采取常规教育策略,于此基础上观察组的144例采取基于某宣教平台的健康教育,干预时间4个月。观察干预前后两组心理弹性(CD-RISC)、自我效能感、健康行为(HPLP)及导管相关并发症情况。结果干预2个月、4个月后,观察组CD-RISC评分分别为(73.72±6.10)分和(92.38±7.24)分,高于对照组的(51.47±6.85)分和(75.20±4.98)分(P<0.05);干预4个月后观察组自我效能感低水平、中等水平和高等水平比例分别为1.92%、55.77%和42.31%,优于对照组的低水平(15.38%)、中等水平(61.54%)和高等水平(23.08%)(P<0.05);干预2个月、4个月后,观察组HPLP量表健康责任评分分别为(13.47±2.00)分和(15.98±2.24)分、营养分别为(13.87±2.12)分和(17.12±3.47)分、压力管理分别为(16.01±3.11)分和(19.00±2.65)分、躯体活动分别为(11.98±1.38)和(1565±2.36)分,均高于对照组HPLP量表健康责任的(12.01±2.36)和(13.69±2.89)分、营养的(11.57±1.87)和(13.44±2.14)分、压力管理的(13.95±2.47)和(16.01±3.00)分、躯体活动的(8.69±1.24)和(10.87±1.58)分(P<0.05);观察组导管相关并发症发生率8.33%低于对照组29.37%(P<0.05)。结论通过某宣教平台对PICC置管肿瘤患者展开有计划、全面、系统且形式丰富的导管维护宣教,可提高患者自我效能感,改善心理弹性水平,促进健康行为,从而降低并发症发生率。 Objective To investigate the effects of health education based on the Huiyuan mission platform on the psychological resilience,self-efficacy and catheter-related complications of PICC-inserted tumor patients,and to provide a basis for clinical improvement of health education strategies.Methods 287 patients with PICC intubation tumors in our hospital from January-December 2021 were selected and randomly divided into groups.143 patients in the control group adopted conventional education strategies.On this basis,the observation group adopted health education based on the Huimei education platform.The intervention time was 4 months.The psychological resilience(CD-RISC),self-efficacy,health behavior(HPLP)and catheter-related complications of the two groups before and after the intervention were observed.Results After 2 and 4 months of intervention,the CD-RISC scores were higher in the observation group(73.72±6.10)and(92.38±7.24)than in the control group(51.47±6.85)and(75.20±4.98)(P<0.05);after 4 months of intervention,self-efficacy was higher in the observation group(1.92%at low level,55.77%at medium level,42.31%at high level)than in the control group(15.38%at low level,61.54%at medium level,23.08%at high level)(P<0.05).42.31%than the control group(15.38%at low level,61.54%at moderate level,23.08%at high level)after 4 months of intervention(P<0.05);After 2 months and 4 months of intervention the observation group had higher HPLP scale health responsibility(13.47±2.00),(15.98±2.24),nutrition(13.87±2.12),(17.12±3.47),stress management(16.01±3.11),(19.00±2.65),somatic activity(11.98±1.38),(15.65±2.36)scores were higher than the control group HPLP scale health responsibility(12.01±2.36),(13.69±2.89),nutrition(11.57±1.87),(13.44±2.14),stress management(13.95±2.47),(16.01±3.00),physical activity(8.69±1.24),(10.87±1.58),and(10.87±1.58)points(P<0.05);the incidence of catheter-related complications was lower in the observation group at 8.33%than in the control group at 29.37%(P<0.05).Conclusion The planned,comprehensive,systematic and rich forms of catheter maintenance education for PICC catheterized tumor patients through Huimei education platform can improve patients'self-efficacy,improve psychological resilience,promote healthy behaviors,and reduce the incidence of complications.
