子宫肌瘤病种门诊与住院服务相关性研究

Study on Correlation Between Outpatient and Inpatient Services of Uterine Fibroids

目的以子宫肌瘤病种为对象研究门诊和住院服务相关性,为控制医疗总费用提供数据支持。方法选取北京市某妇产医院2015年1月1日-2017年12月31日19341名门诊子宫肌瘤患者,以及2510名门诊且住院的子宫肌瘤患者为研究对象,引用两部模型原理,对门诊和住院服务利用分为两部分进行相关性分析。结果样本数据中,门诊次均608元,就诊次数平均2.257次,住院次均11795元,其中门诊总费用、门诊就诊人次、年龄、疾病合并症等因素能增加住院概率;门诊总费用、门诊基金支付等因素能增加住院总费用;门诊就诊次数对住院费用和住院天数并无影响。结论子宫肌瘤患者的疾病负担和经济负担不容忽视,且门诊和住院服务利用一定程度上呈互补关系,建议对于手术类疾病,控制门诊就诊次数,可降低整体医疗费用。

Objective It was of great significance to control the total medical expenses to study the correlation between outpatient service and inpatient service with uterine fibroids.Methods 19,341 outpatient patients with uterine fibroids in a gynecology and obstetrics hospital in Beijing from 2015 to 2017 and 2510 outpatient and inpatient patients with uterine fibroids were selected as the research objects.Two models were used to analyze the correlation between outpatient and inpatient services.Results In the sample data,the average outpatient visit was 608 yuan,the average outpatient visit was 2.257 times,and the average hospitalization visit was 11,795 yuan.Factors such as total outpatient expenses and outpatient fund payment can increase the total hospitalization expenses;The number of outpatient visits had no effect on the cost and length of stay.Conclusions The disease burden and economic burden of patients with uterine fibroids should not be ignored,and the utilization of outpatient and inpatient services was complementary to a certain extent.It was suggested that for surgical diseases,controlling the number of outpatient visits could reduce the overall medical cost.
