急性淋巴细胞白血病初治患儿住院费用及影响因素分析

Analysis of Hospitalization Expenses and Influencing Factors in Children with Newly Diagnosed Acute Lymphoblastic Leukemia

目的分析急性淋巴细胞白血病初治患儿住院费用水平及构成,探讨住院费用的影响因素。方法选取某三甲医院2015年1月1日-2017年12月31日收治的249例急性淋巴细胞白血病初治患儿,经纳入和排除后选取219例患者,对住院费用影响因素进行非参数检验、多元线性回归分析。结果患儿年均住院费用为75950.32元,次均住院费用为11329.41元,日均住院费用为912.87元。患儿年均、次均和日均药费分别为38823.95元、5821.33元、472.90元,在各分项费用最高,其次为化验费、卫材费。在分项费用占比中,药费占比最大,为54.23%,其次为化验费、治疗费,分别占17.3%、6.87%。结果显示住院时间(周)、住院次数、病理分期、治疗阶段与住院费用之间的差异有统计学意义。结论住院时间(周)、住院次数、病理分期、治疗阶段是影响患儿住院费用的主要因素。

Objective To analyze the level and composition of hospitalization expenses of children with newly diagnosed acute lymphoblastic leukemia,and to explore the influencing factors of hospitalization expenses.Methods 249 children with newly diagnosed acute lymphoblastic leukemia admitted to a top three hospital from January 1,2015 to December 31,2017.219 patients were selected after inclusion and exclusion,non-parametric test and multiple linear regression analysis of factors affecting hospitalization expenses.Results The average annual hospitalization cost of children was 75950.32 yuan,the average hospitalization expenses were 11329.41 yuan,and the average daily hospitalization expenses were 912.87 yuan.The average annual,average and average daily drug costs of children were 38,823.95 yuan,5,821.33 yuan,and 472.90 yuan.The cost of medicines is the highest in each item,followed by laboratory and health materials.Among the proportion of sub-items,the proportion of medicines was the largest,at 54.23%,followed by laboratory fees,and health materials fees accounted for 17.30%and 6.87%.The results showed statistically significant differences between hospital stay(week),number of hospitalizations,pathological stage,treatment stage,and hospitalization costs.Conclusions Among the hospitalization expenses,the average cost and proportion of medicine,laboratory and medical materials were relatively high;the hospital stay(week),number of hospitalizations,pathological stage,and treatment stage were the main factors affecting the hospitalization expenses of children.
