MRI诊断膝关节外伤后交叉韧带损伤的临床研究

Clinical study of MRI in the diagnosis of cruciate ligament injury after knee joint trauma

目的探讨MRI诊断膝关节外伤后前后交叉韧带损伤的临床价值。方法收集2017年1月-2018年12月东莞市虎门医院收治的166例膝关节外伤后前后交叉韧带损伤患者为本次研究对象,曾在外院就诊采用CT诊断93例为CT组,于我院就诊采用MRI诊断73例为MRI组。比较两组诊断结果。结果CT组前交叉韧带(ACL)、后交叉韧带(PCL)测量值与金标准比较差异有统计学意义(P<0.05),MRI组ACL、PCL测量值与金标准比较差异无统计学意义(P>0.05),MRI测量值明显更接近金标准。CT组诊断符合87例,符合率为93.55%;MRI组诊断符合73例,符合率为100.00%,差异有统计学意义(P<0.05)。结论MRI诊断膝关节外伤后前后交叉韧带损伤结果与关节镜金标准更接近,可以作为无创诊断的首选方法。

Objective To explore the clinical value of MRI in the diagnosis of anterior and posterior cruciate ligament injury after knee joint trauma.Methods From January 2017 to December 2018,166 cases of anterior and posterior cruciate ligament injury after knee joint injury were collected for the study objects,93 cases diagnosed by CT in other hospital were CT group and 73 cases diagnosed by MRI in our hospital were MRI group.The diagnosis results of the two groups were compared.Results There were significant differences between anterior crucitae ligament(ACL),posterior cruciate ligament(PCL)measured value and gold standard in CT group(P<0.05).There were no significant differences between ACL,PCL measured value and gold standard in MRI group(P>0.05).The MRI measured values were significantly closer to the gold standard.In the CT group,87 cases were diagnosed and the coincidence rate was 93.55%.In the MRI group,73 cases were diagnosed and the coincidence rate was 100.00%.The differences were statistically significant(P<0.05).Conclusion MRI diagnosis of anterior and posterior cruciate ligament injury after knee joint trauma was closer to the arthroscopic gold standard,which could be used as the first choice method of noninvasive diagnosis.
