2011—2020年宁夏回族自治区盐池县高危孕产妇管理情况分析

Analysis on the management of high-risk pregnant women in Yanchi County,Ningxia Hui Autonomous Region from 2011 to 2020

目的回顾性总结2011—2020年宁夏回族自治区盐池县高危孕产妇管理情况,分析高危孕产妇管理过程中存在的问题,为提出有针对性的干预措施提供依据。方法选取2011—2020年宁夏回族自治区盐池县高危孕产妇管理系统中的高危孕产妇为研究对象。对高危孕产妇发生率、孕产妇死亡率、围产儿死亡率等数据进行统计分析,组间比较采用χ^(2)检验和趋势χ^(2)检验。结果2011—2020年宁夏回族自治区盐池县孕产妇的住院分娩率、早孕建卡率、产后访视率、系统管理率较为稳定,系统管理率一直保持在95.0%~97.9%,均达标(95.0%以上)。2011—2020年盐池县高危孕产妇发生率呈逐年上升趋势(χ_(趋势)^(2)=1283.779,P<0.001);“五色管理”措施实施前、后围产儿死亡率分别为12.65‰和7.55‰,差异有统计学意义(χ^(2)=12.037,P=0.001)。结论规范高危孕产妇过程及环节的管理,加大高危孕产妇管理力度,重视健康教育,促进高危孕产妇管理质量的提升,是降低高危孕产妇发生率、孕产妇死亡率及围产儿死亡率的关键。

Objective To summarize the management of high-risk pregnant women in Yanchi County,Ningxia Hui Autonomous Region from 2011 to 2020 retrospectively,and analyze the problems existing in the process of management,so as to provide evidence for targeted intervention.Methods All high-risk pregnant women in Yanchi County,Ningxia Hui Autonomous Region management system from 2011 to 2020 were recruited as research subjects.The incidence of high-risk pregnant women,maternal mortality,and perinatal mortality were analyzed.χ^(2)test and trendχ^(2)test were used for comparisons between groups.Results The rates of hospital delivery,early pregnancy registry,postpartum visit,and system management in Yanchi County,Ningxia Hui Autonomous Region were stable from 2011 to 2020.The system management rate had been maintained at 95.0%-97.9%in the past 10 years,which all met the national standard(95.0%above).The incidence of high-risk pregnant women showed an upward trend year by year(χ_(trend)^(2)=1283.779,P<0.001).Perinatal mortality rates pre and post implementation of the“Five Color Management”measure were 12.65‰and 7.55‰,and the changes were significantly different(χ^(2)=12.037,P=0.001).Conclusion Process and stage management of high-risk pregnant women should be standardized and reinforced,and health education should be emphasized to improve the quality of management,which are the key to reduce the incidence of high-risk pregnant women,maternal mortality rate,and perinatal mortality.
