下肢动脉闭塞球囊扩张术与胫骨横向骨搬移微循环重建技术治疗糖尿病足的疗效对比

Comparison of the efficacy of lower limb artery occlusion and tibial microcirculation reconstruction in the treatment of diabetic foot

目的对比下肢动脉闭塞球囊扩张术与胫骨横向骨搬移微循环重建技术治疗糖尿病足的有效性和安全性。方法选取2021年6月至2022年6月郑州人民医院收治的160例糖尿病足患者作为研究对象,按随机数字表法分为重建技术组、球囊扩张术组,重建技术组给予胫骨横向骨搬移微循环重建技术治疗,球囊扩张术组给予下肢动脉闭塞球囊扩张术治疗;检测踝肱指数、患肢皮温、感觉阈值、疼痛视觉模拟评分(visual analogue scale,VAS)、密歇根神经病变筛选表(Michigan neuropathy screening instrument,MNSI)评分及血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)、碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)水平的变化情况,并统计并发症发生率以及临床疗效评价。结果与重建技术组相比,球囊扩张术组的踝肱指数、患肢皮温及VEGF,bFGF水平较高(P<0.05);与重建技术组相比,球囊扩张术组的感觉阈值及VAS评分、MNSI评分降低,差异有统计学意义(P<0.05)。结论下肢动脉闭塞球囊扩张术效果优于胫骨横向骨搬移微循环重建技术,可有效减少并发症,促进术后快速恢复。

Objective The efficacy and safety of lower limb artery occlusion and tibial microcirculation reconstruction in the treatment of diabetic foot.Methods Selected the 160 diabetic foot patients admitted to Zhengzhou People’s Hospital from June 2021 to June 2022,According to the random number table,it was divided into the reconstruction technique group and balloon dilation group,The reconstruction technique group was treated with the microcirculation reconstruction technique,Balloon dilation group received balloon dilation of lower extremity artery occlusion;Ankle brachial index,skin temperature,sensory threshold,visual analogue scale(VAS),Michigan neuropathy screening instrument(MNSI)and vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)levels,The complication rate and the clinical efficacy evaluation were also counted.Results Ankle brachial index,VEGF,and bFGF levels were higher in the balloon dilation group(P<0.05),compared with the reconstruction technique group,the sensory threshold,VAS score and MNSI score were lower in the balloon dilation group,statistically significant(P<0.05).Conclusion It is better than the tibia,which can effectively reduce complications and promote rapid postoperative recovery.
