子宫内膜癌患者血清脂质运载蛋白-2、内脂素水平变化及临床意义

Changes and clinical significance of serum lipocalin-2 and lipolipin levels in patients with endometrial cancer

目的探讨子宫内膜癌(endometrial cancer)患者的血清脂质运载蛋白-2(lipocalin-2,LCN-2)、内脂素(visfatin)的水平变化及临床意义。方法选取2020年1月至2022年1月西南医科大学附属中医医院妇科收治的子宫内膜癌患者156例(试验组),另选健康体检者80名(对照组),采用酶联免疫吸附法检测各组受试者的血清LCN-2、visfatin水平;分析血清LCN-2、visfatin与患者的病理类型、肿瘤直径、国际妇产科联盟国际妇产科联盟(International Federation of Gynecology and Obstetrics,FIGO)分期、病理分化等病理特征的关系;采用受试者操作特征曲线(receiver operating characteristic,ROC),分析血清LCN-2、visfatin对子宫内膜癌发生的预测价值。结果试验组患者的血清LCN-2、visfatin水平均比对照组高(P<0.05)。子宫内膜癌患者不同体质量指数(body mass index,BMI)、FIGO分期、病理分化、淋巴结转移、血管浸润血清LCN-2、visfatin水平相比,差异有统计学意义(P<0.05)。ROC曲线显示:血清LCN-2、visfatin联合检测预测,子宫内膜癌发生的曲线下面积为0.879,敏感性和特异性为0.893和0.886。多因素logistic回归分析示:血清糖类抗原125(carbohydrate antigen125,CA125)、糖类抗原199(carbohydrate antigen 199,CA199)、LCN-2、visfatin水平变化是患者发生子宫内膜癌的独立危险因素(P<0.05)。结论子宫内膜癌患者血清LCN-2、visfatin水平升高,血清LCN-2、visfatin水平与患者的临床病理特征有关,对预测子宫内膜癌的发生发展有重要的临床意义。

Objective To investigate the changes and clinical significance of serum lipocalin-2(LCN-2)and visfatin in patients with endometrial cancer.Methods From January 2020 to January 2022,156 patients with endometrial cancer treated in department of gynaecology from Southwest Medicial University affiliated Traditional Chinese Medicine Hospital(experimental group)and 80 healthy subjects(control group)were selected.Detection of serum LCN-2 and visfatin levels in each group by enzyme-linked immunosorbent assay.The relationship between serum LCN-2 and visfatin and pathological characteristics such as pathological type,tumor diameter,FIGO stage and pathological differentiation were analyzed.The ROC curve was used to analyze the predictive value of serum LCN-2 and visfatin on the occurrence of endometrial cancer.Results Serum levels of LCN-2 and visfatin in experimental group were higher than those in healthy control group(P<0.05).Endometrial cancer patients had different BMI,FIGO staging,pathological differentiation,lymph node metastasis,vascular infiltration serum LCN-2,and visfatin levels,the differences were statistically significant(P<0.05).The ROC curve showed that the combined detection of serum LCN-2 and visfatin predicted the area under the curve of endometrial cancer to be 0.879,and the sensitivity and specificity were 0.893 and 0.886.Multivariate logistic regression analysis showed that changes in serum CA125,CA199,LCN-2 and visfatin levels were independent risk factors for endometrial cancer(P<0.05).Conclusions Serum levels of LCN-2 and visfatin in patients with endometrial cancer are elevated.Serum levels of LCN-2 and visfatin are related to the clinicopathological characteristics of patients and have important clinical significance for predicting the occurrence and development of endometrial cancer.
