上消化道症状就诊人群幽门螺杆菌感染调查分析

Investigation and analysis of Helicobacter pylori infection in patients with upper gastrointestinal tract symptoms

目的调查社区卫生服务中心有上消化道症状的就诊人群幽门螺旋杆菌(Helicobacter pylori,Hp)感染情况和相关危险因素。方法选择2021年7—8月于北京市朝阳区团结湖、劲松、大屯、香河园社区卫生服务中心和北京京北医院不论何种原因就诊的18~80岁有上消化道症状的就诊患者,进行Hp筛查和调查问卷。结果360例被调查者中,Hp感染者213例(59.17%)。有口臭者的Hp感染率高于无此症状者,有反酸者的Hp感染率低于无此症状者。有3种及以上消化道症状者Hp感染率(76.71%)高于有一种消化道症状者(43.56%)。Logistic回归分析发现,抑郁、糖尿病、吸烟史、既往胃病史、宵夜习惯、就餐不规律与Hp感染有关。结论上消化道症状越多,Hp感染率越高;抑郁状态、糖尿病、吸烟史、既往胃病史、宵夜习惯、就餐不规律可能与Hp感染有关。

Objective This study was aimed at investigating Helicobacter pylori(Hp)infection and related risk factors among patients with upper digestive tract symptoms in community health service centers.Method Hp screening and a questionnaire survey were performed on patients 18-80 years old with upper digestive tract symptoms who were admitted to The Community Health Service Center of Tuanjiehu,Jinsong,Datun,Xiangheyuan in Chaoyang District,and Jingbei Hospital of Beijing for any reason from July to August of 2021.Results Amon gall 360 cases,Hp infection was found in 213 cases(59.17%).Hp infection rate in patients with halitosis was higher than that in patients without halitosis.Hp infection rate in patients with acid reflux was lower than that in patients without acid reflux.The rate of having more than three digestive tract symptoms(76.71%)was greater than the rate of having one digestive tract symptom(43.56%).Logistic regression analysis indicated depressive,diabetes,smoking history,past history of stomach disease,night snacking habits and irregular meal consumption were associated with Hp infection.Conclusion More upper gastrointestinal symptoms are associated with a higher Hp infection rate.Depression,diabetes,smoking history,past history of stomach disease,night snacking habits and irregular meal consumption may be associated with Hp infection,and therefore should receive clinical attention.
