美托洛尔联合依那普利对慢性心力衰竭患者心功能及血核转录因子κB水平的影响

Effects of metoprolol combined with enalapril on cardiac function and blood nuclear transcription factor-κB levels in patients with chronic heart failure

目的探讨美托洛尔联合依那普利对慢性心力衰竭患者心功能及血核转录因子κB(nuclear transcription factor-κB,NF-κB)水平的影响。方法选取2018年3月至2019年3月安阳市第三人民医院就诊的100例慢性心力衰竭患者作为研究对象,采用随机数字表法分为对照组与观察组,每组50例。对照组给予美托洛尔治疗,观察组给予美托洛尔联合依那普利治疗。比较两组治疗3个月后的疗效、心功能指标、血NF-κB水平,比较两组患者用药期间不良反应的发生情况。结果治疗3个月后,观察组的总有效率为90.00%,高于对照组的74.00%,差异有统计学意义(P<0.05);且观察组心输出量(cardiac output,CO)、每搏输出量(stroke volume,SV)、左室射血分数(left ventricular ejection fraction,LVEF)高于对照组,差异有统计学意义(P<0.05),左心室舒张内径(left ventricular diastolic diameter,LVDD)低于对照组,差异有统计学意义(P<0.05);观察组血NF-κB水平低于对照组,差异有统计学意义(P<0.05)。用药期间两组患者不良反应发生情况比较,差异无统计学意义(P>0.05)。结论在对慢性心力衰竭患者进行治疗时,应用美托洛尔联合依那普利治疗能够改善患者的心功能状态,调节血NF-κB水平,疗效显著,且安全性较高。

Objective To explore the effects of metoprolol combined with enalapril on cardiac function and blood nuclear transcription factor-κB(NF-κB)levels in patients with chronic heart failure.Methods A total of 100 patients with chronic heart failure who were treated in our hospital from March 2018 to March2019 were selected and divided randomly into control group and observation group(50 patients in each group).The control group was treated with metoprolol and the observation group was given metoprolol combined with enalapril.The clinical efficacy,cardiac function indexes and blood NF-κB levels were compared between the two groups after 3 months of treatment.Adverse reactions during treatment were also compared between the two groups.Results The overall efficacy rate was significantly higher in the observation group(90.00%)compared with the control group(74.00%),the difference was statistically significant(P<0.05).Cardiac output,stroke volume and left ventricular ejection fraction were all significantly higher and left ventricular diastolic diameter was significantly lower in the observation group compared with the control group,the differences were statistically significant(P<0.05).Blood NF-κB level was significantly lower in the observation group than in the control group,the difference was statistically significant(P<0.05).The occurrence of adverse reactions was similar in both groups during treatment,the difference was statistically significant(P>0.05).Conclusion Metoprolol combined with enalapril can improve the cardiac function status and regulate blood NF-κB levels in patients with chronic heart failure,with significant efficacy and good safety.
