缺血性卒中患者亚甲基四氢叶酸还原酶基因多态性和血清同型半胱氨酸水平变化及意义 被引量:2

Changes and significance of methylenetetrahydrofolate reductase gene polymorphism and serum homocysteine levels in patients with ischemic stroke

摘要 目的探讨缺血性卒中患者亚甲基四氢叶酸还原酶(MTHFR)基因多态性、血清同型半胱氨酸(Hcy)表达水平变化及意义。方法选取2019年1月至2020年12月于聊城市第三人民医院就诊的110例首次发病的缺血性卒中患者(观察组)及110名健康体检人群(对照组)作为研究对象。比较两组MTHFR C677T基因型和血清Hcy的差异,并分析MTHFR C677T基因型、Hcy与卒中程度及类型的关系。结果观察组CC基因型低于对照组,TT基因型高于对照组,差异有统计学意义(P<0.05),两组CT基因型比较,差异无统计学意义(P>0.05);等位基因显示,观察组C等位基因低于对照组,T等位基因高于对照组,差异有统计学意义(P<0.05)。观察组血清Hcy平均水平高于对照组,差异有统计学意义(P<0.05)。两组TT基因型Hcy水平均显著高于同组CC、CT型,差异有统计学意义(P<0.05),而CT与CC型之间比较,差异无统计学意义(P>0.05)。卒中轻度、中度、重度组间MTHFR C677T分型和Hcy比较,差异无统计学意义(P>0.05)。卒中前循环、后循环间MTHFR C677T分型和Hcy比较,差异无统计学意义(P>0.05)。结论MTHFR C677T基因型、T等位基因频率及血清Hcy在缺血性卒中患者中具有较高表达水平,但与卒中程度和卒中类型无关。 Objective To investigate the polymorphism of methylenetetrahydrofolate reductase(MTHFR)and serum homocysteine(Hcy)level in patients with ischemic stroke.Methods A total of 110 patients with first-onset ischemic stroke(observation group)and 110 healthy controls(control group)from Jan 2019 to Dec 2020 were selected in Liaocheng Third People’s Hospital.The differences of MTHFR C677 T genotype and serum Hcy were compared between the two groups.Then,relationships between MTHFR,Hcy and the degree and type of stroke were analyzed.Results The CC genotype in the observation group was lower than that in the control group(P<0.05),and the TT genotype was higher in the observation group(P<0.05).There was no significant difference in CT genotype between the two groups(P>0.05).The C allele in the observation group was lower than that in the control group,and the T allele was higher in the observation group(P<0.05).The serum Hcy level in the observation group was higher than that in the control group(P<0.05).In both groups,the Hcy of TT genotype was significantly higher than that of CC or CT genotype(P<0.05),but the difference between CT and CC was not statistically significant(P>0.05).There was no significant difference in MTHFR C677 T classification and Hcy level among mild,moderate and severe stroke groups(P>0.05).There was no significant difference in MTHFR C677 T classification and Hcy level between anterior circulation and posterior circulation stroke(P>0.05).Conclusions MTHFR C677 T genotype,T allele frequency and serum Hcy levels are higher in patients with ischemic stroke,but they are not related with the degree or type of stroke.
