新型冠状病毒肺炎疫情期间高血压患者治疗依从性现况及相关因素

Treatment compliance and related factors of patients with hypertension during the epidemic period of COVID-19

目的探讨新型冠状病毒肺炎(以下简称新冠肺炎)疫情期间高血压患者治疗依从性的影响因素,为开展高血压患者远程管理提供依据。方法选取2020年2月至2020年4月福建省立医院收治的268例高血压患者,采用自行编制的《高血压患者治疗依从性量表》对患者进行调查,得出遵医服药行为、不良服药行为、日常生活管理、新冠肺炎自我防护四大维度得分,分析维度之间关系,比较不同特征高血压患者之间治疗依从性的差异。结果遵医服药行为得分(21.53±3.94)分,不良服药行为得分(27.53±5.83)分,日常生活管理得分(40.85±8.79)分,新冠肺炎自我防护得分(16.09±3.18)分,总得分(106.00±18.48)分,四大维度之间呈正相关;不同性别、是否患有其他慢性病的高血压患者治疗依从性比较,差异无统计学意义(P>0.05),不同居住地、文化水平、家庭人均月收入组间高血压患者治疗依从性比较,差异有统计学意义(P<0.05)。结论新冠肺炎疫情期间高血压患者治疗依从性较低,尤其是居住在农村、文化水平低、家庭人均月收入低者应被纳入重点管理对象,保证该类患者血压控制达标。

Objective To investigate the status of treatment compliance of hypertensive patients in novel coronavirus disease 2019(COVID-19)outbreak,analyze the relevant influencing factors,and provide a basis for remote management of hypertensive patients.Methods A total of 268 patients with hypertension were selected from February 2020 to April 2020 in Fujian provincial hospital.A survey was conducted using the self-made treatment compliance scale which included four items such as the medical treatment behavior,bad medicine behavior,daily life management,COVID-19 self-protection.The relationship of the four dimensions was analyzed and the difference of hypertension treatment adherence among different characteristics of patients were compared.Results The medical treatment behavior score was 21.53±3.94;the bad medicine behavior score was 27.53±5.83;the daily life management score was 40.85±8.79;the COVID-19 self protection score was 16.09±3.18;the total score was106.00±18.48.The four dimensions were positively correlated.There were no significant differences in treatment adherence between different gender and groups suffering from other chronic diseases or not(P>0.05).There were statistically significant differences in treatment compliance between different residence,education level and family per capita monthly income(P<0.05).Conclusion The treatment compliance of hypertensive patients is relatively low during the outbreak of COVID-19,especially those living in rural areas with low educational level and low family per capita monthly income,which should be included into the key management objects in order to control their blood pressure well.
