治疗相关髓系肿瘤10例临床特征

Clinical characteristics of 10 patients with treatment related myeloid tumors

目的探讨治疗相关髓系肿瘤的临床特征,为临床治疗提供参考。方法收集内蒙古自治区人民医院2014年1月—2019年1月诊治的10例治疗相关髓系肿瘤患者的临床资料进行分析。结果10例患者在诊断前均接受过针对原发肿瘤的联合化疗和(或)放疗,其中治疗相关急性髓系白血病7例,治疗相关骨髓增生异常综合征(MDS)3例。1例治疗相关急性早幼粒细胞白血病患者在确诊4 d内死亡,1例老年MDS仅刺激造血及维持治疗,于随访22个月时死亡;其余8例均接受规律化疗,4例患者疗效达到完全缓解,4例在治疗12~18个月时死亡。结论治疗相关髓系肿瘤生存期短、治疗效果差,需要采用新药/造血干细胞移植治疗以改善疗效。

Objective To explore the clinical characteristics of the treatment related myeloid tumors,and to provide reference for clinical treatment.Methods Clinical data of 10 patients with treatmentrelated myeloid tumor diagnosed in Inner Mongolia People’s Hospital from January 2014 to January2019 were collected and analyzed.Results All these 10 patients had received combined chemotherapy and radiotherapy for primary tumors.Among them,7 cases were treatment-related acute myeloid leukemia(AML)and 3 cases were treatment-related myelodysplastic syndrome(MDS).One patient with acute promyelocytic leukemia(APL)died within 4 days after diagnosis,and one elderly patient with MDS received maintenance treatment and died at a 22-month follow-up.The other 8 patients received regular chemotherapy,4 of them achieved complete remission,while 4 other cases died at 12-18 months of treatment.Conclusion The short survival and poor therapeutic effect of the treatment of related myeloid tumors require the use of new drugs/hematopoietic stem cell transplantation to improve the therapeutic effect.
