独圣活血片、丁丙诺啡透皮贴联合神经阻滞用于胸椎压缩性骨折急性期镇痛的临床观察

Clinical Observation on Dusheng Huoxue Tablets and Buprenorphine Transdermal Patch Combined with Nerve Block in Analgesia for Compression Fracture of Thoracic Vertebrae in Acute Stage

目的:探讨独圣活血片、丁丙诺啡透皮贴联合神经阻滞用于胸椎压缩性骨折急性期的镇痛效果。方法:选取2016年1月至2019年5月遂宁市中心医院收治的胸椎压缩性骨折伴剧烈疼痛患者190例,采用随机数字表法分为A、B组,每组95例。A组患者采用独圣活血片、丁丙诺啡透皮贴联合神经阻滞进行镇痛治疗,B组患者采用神经阻滞联合非甾体抗炎药进行镇痛治疗。观察两组患者治疗前,治疗后24 h、48 h、72 h及7 d的疼痛分级指数,以及静息和运动时的疼痛视觉模拟评分(visual analog score,VAS),并观察药品不良反应。结果:治疗48、72 h及7 d后,A组患者的疼痛分级指数明显低于B组,差异均有统计学意义(P<0.05)。治疗48、72 h及7 d后,A组患者静息和活动时的VAS评分明显低于B组,差异均有统计学意义(P<0.05)。A组患者无明显不良反应发生,B组15例患者出现轻微的消化道反应,不良反应发生率为15.79%。结论:独圣活血片、丁丙诺啡透皮贴联合神经阻滞对胸椎压缩性骨折急性疼痛的镇痛效果确切,安全性较好。

OBJECTIVE:To probe into the analgesia effects of Dusheng Huoxue tablets and buprenorphine transdermal patch combined with nerve block for compression fracture of thoracic vertebrae in acute stage.METHODS:Totally 190 patients with compression fracture of thoracic vertebrae in acute stage admitted into Suining Central Hospital from Jan.2016 to May 2019 were extracted to be divided into the group A and the group B via the random number table,with 95 cases in each group.The group A was treated with Dusheng Huoxue tablets and buprenorphine transdermal patch combined with nerve block for analgesia,while the group B received nerve block combined with nonsteroidal anti-inflammatory and analgesic drugs.The pain rating index and visual analogue score(VAS)in resting state and motion state was evaluated before treatment and after treatment of 24 h,48 h,72 h and 7 d.The incidence of adverse drug reactions was observed.RESULTS:After treatment of 48 h,72 h and 7 d,the pain rating index in the group A was significantly lower than that in the group B,with statistically significant difference(P<0.05).After treatment of 48 h,72 h and 7 d,the VAS in resting state and motion state in the group A was significantly lower than that in the group B,with statistically significant difference(P<0.05).There were no obvious adverse drug reactions in group A,while there were 15 patients(15.79%)with mild gastrointestinal reactions in group B.CONCLUSIONS:The analgesic effects of Dusheng Huoxue tablets and buprenorphine transdermal patch combined with nerve block on acute pain caused by compression fracture of thoracic vertebrae is definite,and the treatment is relatively safer.
