Trend analysis of infectious disease spectrum in an army from 2009 to 2018

摘要 目的了解2009-2018年某部传染病疾病谱变化趋势,掌握某部传染病发病情况、流行规律,为制定有效的防控策略提供依据。方法利用军队疫情直报系统和Excel、SPSS 19. 0软件对2009-2018年某部传染病疾病谱进行统计分析。结果 2009-2017年某部传染病发病在600~800例之间,总体呈上下窄幅波动的平稳态势,平均数x为710例,s=47. 22,2018年报告发病1 200余例,差异有统计学意义(χ^2=144. 40,P <0. 01)。报告发病居前5位的病种主要为肺结核、水痘、病毒性肝炎、流行性感冒、其它感染性腹泻;以呼吸道传染病居首,按月统计,发病最少的为三季度,其中最少的7月,占发病总数的7. 08%,发病最多的为一季度,其中最多的为1月,占发病总数的9. 41%;发病人员中士兵发病最多,其次为干部;上报突发公共卫生事件64起,其中传染病62起,占96. 87%,食物中毒2起,占3. 13%。结论近10年来某部传染病疾病谱变化显著,与2001-2010年某部疾病谱有明显差别,呼吸道传染病、血源及性传播疾病发病的上升以及肠道传染病的下降,与全军整体疫情变化趋势一致。 Objective To understand the changing trend of disease spectrum of infectious diseases in an army from 2009 to2018. To master the incidence and epidemic law of infectious diseases in an army,and provide a basis for formulating effective prevention and control strategies. Methods The military epidemic situation direct reporting system,Excel and SPSS 19. 0 software were used for the statistical analysis of the infectious disease spectrum of an army from 2009 to 2018. Results It was reported the incidence of infectious diseases in an army ranged from 600 to 800 cases from 2009 to 2017,showing a stable fluctuation in a narrow range,with an average of 710 cases,s = 47. 22,and more than 1 200 cases reported in 2018,with statistically significant differences( χ^2= 144. 40,P < 0. 01). Tuberculosis,chicken pox,viral hepatitis,influenza,and other infectious diarrhea were the top 5 diseases reported. Respiratory infectious diseases led the way. According to the monthly statistics,the incidence was the least in the third quarter,among which July was the least,accounting for 7. 08% of the total incidence. The incidence was the most in the first quarter,and January was the most,accounting for 9. 41% of the total incidence.Among the sick people,the soldiers had the most morbidity,followed by the officers. 64 public health emergencies were reported,among which 62 were infectious diseases,accounting for 96. 87%,and 2 were food poisoning,accounting for 3. 13%. Conclusion In the past 10 years,the disease spectrum of infectious disease in an army has significantly changed,which is significantly different from that of an army from 2001 to 2010. The incidence of respiratory infectious diseases,blood-borne diseases and sexually transmitted diseases has increased,and the incidence of intestinal infectious diseases has decreased,which is consistent with the trend of the overall epidemic situation in the PLA.
