加味柴胡疏肝散治疗螺内酯致男性乳腺增生及对血浆黄体生成素、促卵泡激素水平影响研究

Clinical Study on the Treatment of Male Mammary Gland Hyperplasia Induced by Spironolactone and the Changes of Luteinizing Hormone and Follicle-stimulating Hormone Levels with Modified Chaihu Shugan Powder

目的:观察加味柴胡疏肝散对螺内酯治疗心力衰竭中引起男性乳腺增生的临床疗效及其对血浆黄体生成素(LH)、促卵泡激素(FSH)水平的影响。方法:将螺内酯治疗心力衰竭引起男性乳腺增生的患者分为治疗组与对照组,对照组单用他莫昔芬口服,每次10mg,每日2次。治疗组在对照组基础上加服中药加味柴胡疏肝散。治疗2~6个疗程后,观察患者乳房情况(乳房肿块硬度、范围、大小、疼痛程度)及治疗前后血浆LH、FSH水平变化情况。结果:治疗组有效率90.32%,对照组有效率68.96%,治疗组治疗效果明显优于对照组(P<0.05),两组比较差异有统计学意义。比较两组黄体生成素(LH)及促卵泡激素(FSH)水平,治疗组较对照组显著下降,差异具有统计学意义(P<0.05)。结论:加味柴胡疏肝散治疗心力衰竭中螺内酯引起的男性乳腺增生临床疗效显著,可明显降低血浆黄体生成素(LH)及促卵泡激素(FSH)水平,值得临床借鉴。

Objective:To observe the clinical efficacy of modified Chaihu Shugan Powder in the treatment of male breast hyperplasia caused by spironolactone in heart failure and the effect on plasma luteinizing hormone(LH)and follicle-stimulating hormone(FSH)levels.Methods:Spironolactone was used to treat male breast hyperplasia caused by heart failure,and the control group was divided into the treatment group and the control group.Treatment group in the control group on the basis of the addition of traditional Chinese medicine.After two to six courses of treatment,the patients’breast conditions(breast lump size,pain relief degree,length of relief)and changes in LH and FSH levels after treatment were observed.Results:The effective rate of the control group was 68.96%,and that of the treatment group was 90.32%.The curative effect of the treatment group was significantly better than that of the control group.After statistical treatment,P<0.05,the difference between the two groups was statistically significant.Compared with the control group,the levels of luteinizing hormone(LH)and follicle-stimulating hormone(FSH)in the two groups were significantly decreased in the treatment group,and the difference was statistically significant(P<0.05).Conclusion:Modified Chaihu Shugan powder has a significant clinical effect on the treatment of male breast hyperplasia caused by spironolactone in heart failure,and can significantly reduce the plasma luteinizing hormone(LH)and follicle-stimulating hormone(FSH)levels,which is worthy of clinical reference.
