化痰通络汤联合醒脑开窍针刺法对风痰瘀阻证脑梗死急性期患者NIHSS评分及生活质量影响

Effect of Huatan Tongluo Decoction and Xingnao Kaiqiao Acupuncture on NIHSS Score and Quality of Life in Patients with Acute Cerebral Infarction of Wind-phlegm-stasis Syndrome

目的:探讨化痰通络汤联合醒脑开窍针刺法应用于风痰瘀阻证脑梗死急性期患者的效果。方法:选取医院风痰瘀阻证脑梗死急性期患者93例,采用常规西医治疗的45例为对照组,在对照组基础上采用化痰通络汤联合醒脑开窍针刺法治疗的48例为观察组。比较两组疗效,治疗前、治疗14d后血液流变学指标(血浆黏度、高切全血黏度、低切全血黏度),治疗前、治疗后3个月美国国立卫生研究院卒中量表(NIHSS)评分、脑卒中专用生活质量量表(SS-QOL)评分。结果:观察组治疗总有效率高于对照组(P<0.05);治疗14d后,观察组血浆黏度、高切全血黏度、低切全血黏度低于对照组(P<0.05);治疗后3个月,观察组NIHSS评分低于对照组,SS-QOL评分高于对照组(P<0.05)。结论:化痰通络汤联合醒脑开窍针刺法治疗风痰瘀阻证脑梗死急性期患者疗效显著,可改善血液流变学指标,缓解神经缺损症状,提高生活质量。

Objective:To explore the effect of Huatan Tongluo Decoction combined with Xingnao Kaiqiao acupuncture in patients with acute cerebral infarction of wind-phlegm-stasis syndrome.Methods:93 patients with acute cerebral infarction with windphlegm-stasis syndrome in hospital were selected.45 cases treated with conventional western medicine were used as the control group,and 48 cases treated with Huatan Tongluo Decoction combined with Xingnao Kaiqiao acupuncture on the basis of the control group were used as the observation group.The efficacy,hemorheology indexes(plasma viscosity,high shear whole blood viscosity,low shear whole blood viscosity)before and after 14 days of treatment,the National Institutes of Health Stroke Scale(NIHSS)scores,and the Stroke-specific Quality of Life Scale(SS-QOL)scores before and 3 months after treatment were compared between the two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After 14 days of treatment,the plasma viscosity,high shear whole blood viscosity,and low shear whole blood viscosity of the observation group were lower than those of the control group(P<0.05).After 3 months of treatment,the NIHSS score of the observation group was lower than that of the control group,and the SS-QOL score was higher than that of the control group(P<0.05).Conclusion:The combination of Huatan Tongluo Decoction and Xingnao Kaiqiao acupuncture has a significant curative effect in the treatment of acute cerebral infarction patients with wind-phlegm-stasis syndrome,which can improve hemorheology,relieve the symptoms of nerve defect and improve the quality of life.
