巨刺联合PNF康复技术治疗脑卒中后肩手综合征Ⅰ期、Ⅱ期临床研究

Clinical Observation of Giant Needle Combined with PNF Rehabilitation Technique in the Treatment of Shoulder and Hand Syndrome after Stroke

目的:观察巨刺法联合PNF康复技术治疗脑卒中后肩手综合征的临床疗效。方法:将32例脑卒中后肩手综合征患者随机分为治疗组和对照组各16例。治疗组采用巨刺法联合PNF康复技术治疗,对照组采用PNF康复治疗,分别于治疗前后评定两组患者患肢VAS积分、水肿积分、上肢运动功能(FMA)积分,并于治疗结束后评定临床疗效。结果:治疗组总有效率为81.25%,高于对照组的56.25%(P<0.05);治疗后两组患者VAS积分均较治疗前下降(P<0.05),差异具有统计学意义(P<0.05);治疗后两组患者水肿积分均较治疗前下降(P<0.05),但组间比较差异无统计学意义(P>0.05);治疗前两组患者上肢FMA评分比较差异无统计学意义(P>0.05);治疗后两组患者上肢FMA评分均高于治疗前(P<0.05),且治疗组FMA评分高于对照组,差异具有统计学意义(P<0.05)。结论:巨刺法联合PNF康复技术能有效缓解脑卒中后肩手综合征患者的肩部疼痛,减轻患肢肿胀,改善患肢肩部功能,提高患者日常生活活动能力,具有较高的临床疗效,值得临床推广使用。

Objective:To observe the clinical effect of giant needling combined with PNF rehabilitation technology in the treatment of shoulder hand syndrome after stroke.Methods:32 patients with shoulder hand syndrome after stroke were randomly divided into treatment group and control group.The treatment group was treated with giant needling combined with PNF rehabilitation technology,and the control group was treated with PNF rehabilitation.The VAS score,edema score and upper limb motor function(FMA)score of the two groups were evaluated before and after treatment,and the clinical curative effect was evaluated after treatment.Results:The total effective rate in the treatment group was 81.25%,which was higher than 56.25%in the control group(P<0.05);After treatment,the VAS score of the two groups decreased compared with that before treatment(P<0.05),and the difference between the two groups was statistically significant(P<0.05);After treatment,the edema score of the two groups decreased compared with that before treatment(P<0.05),and there was no significant difference between the two groups(P>0.05);There was no significant difference in FMA score of upper limbs between the two groups before treatment(P>0.05);After treatment,the FMA score of the two groups was higher than that before treatment(P<0.05),and the FMA score of the treatment group was higher than that of the control group(P<0.05).Conclusion:giant needling combined with PNF rehabilitation technology can effectively alleviate the shoulder pain of patients with shoulder hand syndrome after stroke,reduce the swelling of affected limbs,improve the function of affected limbs and shoulders,and improve the ability of daily living activities of patients.It has high clinical curative effect and is worthy of clinical promotion.
