从湿毒论治肛周湿疮验案举隅及理论分析

肛周湿疮发病位置比较特殊,居于人体下孔窍,中医认为其为湿邪下注之所,据湿邪的性质,往往缠绵难愈,反复发作。历代医家均注重湿热下注而以四妙散、龙胆泻肝汤治疗,但均忽略湿毒为患,湿毒在局部导致皮肤黏膜的"痒、湿、烂、顽、变"表现,比单纯的湿热下注症状更重、更为顽固,且单纯的口服治疗效果不佳,基于此,应内外结合治疗,尤其要重视外治。程宏斌主任医师认为邪气蕴积谓之毒,肛周湿疮的核心病机在于湿毒留滞孔窍,湿毒致病是其核心病机,治当以燥湿解毒、祛风止痒,自拟七味止痒洗剂祛湿毒,病向愈。以湿毒为切入点,深入论述从湿毒立论治疗该病的理论依据,并列举验案证之,以供同道参考。
