脑小血管性认知障碍临床症状繁多且病因复杂,常合并他症,因此辨证需抓主要病机。周德生教授从荣气虚滞病机阐释,认为脑小血管性认知障碍主要病机特征为位责络脉,玄府失司;正虚邪滞,痰瘀交阻;形神俱病,神机失用。并提出治法:行气活血、通络畅府;滋阴填精、益髓增智;祛瘀化痰、醒脑开窍。强调形神兼顾、杂合以治是治疗该病之核心。并以此为基础创制活血荣络汤,临床应用广泛。

Cerebral small vessel cognitive impairment has many clinical symptoms and complex etiology,often combined with other diseases.So the main pathogenesis should be grasped in syndrome differentiation.Professor Zhou Desheng explained the pathogenesis of cerebral small vessel cognitive impairment from the perspective of Rong Qi deficiency and stagnation.The main pathogenesis characteristics of cerebral small vessel cognitive impairment are:position responsibility and collaterals,Xuanfu loss of administration;deficiency of vital energy and evil stagnation,phlegm and blood stasis obstruction;both body and spirit are ill,and Shenji is out of use.The treatment methods are as follows:promoting qi and activating blood circulation,unblocking collaterals and smoothing the Fu;nourishing yin and filling essence,benefiting marrow and increasing intelligence;removing blood stasis and resolving phlegm,awakening the brain and opening up the mind.The core of the treatment of the disease is to pay attention to both the body and the spirit,and to treat the disease by hybridity.On this basis,Huoxue rongluo decoction was created,which was widely used in clinic.
