脐针治疗中风后脾肾阳虚型尿失禁临床观察

Clinical Study of Umbilicus Acupuncture in the Treatment of Urinary Incontinence of Spleen-Kidney Yang Deficiencyafter Stroke

目的:对脐针治疗中风后脾肾阳虚型尿失禁的临床效果进行评估。方法:采取随机数字表法对90例中风后脾肾阳虚型尿失禁患者进行分组,两组各45例。对照组给予常规中药治疗,观察组加用脐针治疗,对比两组临床疗效、临床症状积分、治疗前后的储尿期最大膀胱测量容量(Maximum bladder measurement capacity,MCBC)、膀胱顺应性(Bladder compliance,BC)、最大逼尿肌压力(Pdetmax)、最大膀胱压(Pvesmax)、排尿期最大尿流率(Qmax)、排尿量(Void volume,Vv)、残余尿量(Relative updated value,RUV)及不良反应。结果:观察组比对照组临床总有效率高(P<0.05)。观察组治疗后与对照组治疗后相比,临床症状积分较低(P<0.05);观察组治疗后与对照组治疗后相比MCBC升高、Pdetmax降低、Pvesmax降低、Vv升高(P<0.05),差异均具有统计学意义。两组治疗期间不良反应发生率无统计学差异(P>0.05)。结论:脐针治疗中风后脾肾阳虚型尿失禁临床疗效显著,可改善临床症状,有效缓解尿动力学相关指标,治疗无不良反应,安全性较高。

Objective:To investigate the clinical effect of umbilical needle in the treatment of urinary incontinence with spleen and kidney Yang deficiency after stroke.Methods:To random number table method in our hospital in March 2018 to March 2020 were of spleen kidney Yang deficiency type of urinary incontinence after stroke patients,90 cases of group,45 cases in each group,control group given conventional treatment,Chinese traditional medicine group with umbilical needle treatment,compared two groups of clinical curative effect,clinical symptoms integral,urine storage period of the Maximum bladder capacity measurement before and after the treatment(Maximum bladder measurement capacity,MCBC),bladder compliance(bladder around,BC),maximum detrusor pressure(Pdetmax),maximum bladder pressure(Pvesmax),maximum urine flow rate(Qmax),Void Volume(Vv),Relative Updated Value(RUV)and adverse reactions.Results:The total effective rate was higher in the observation group than in the control group(P<0.05).After treatment,the clinical symptom score of the observation group was lower than that of the control group(P<0.05);compared with the control group after treatment,the observation group had higher MCBC,lower Pdetmax,lower Pvesmax and higher Vv(P<0.05),and the differences were statistically significant.There was no significant difference in the incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion:Umbilical needle therapy for incontinence with spleen and kidney Yang deficiency after stroke has a significant clinical effect,promotes the improvement of clinical symptoms,effectively alleviates urodynamic related indicators,and has no adverse reactions during treatment,with a high safety.
