基于数据挖掘的针灸治疗慢性心力衰竭选穴规律探讨

Discussion on the Law of Acupuncture and Moxibustion for Chronic Heart Failure Based on Data Mining

目的:通过检索近10年针灸治疗慢性心力衰竭的文献,分析针灸治疗慢性心力衰竭的腧穴配伍特点及规律,对现代临床针灸诊治慢性心力衰竭的腧穴选择规律进行数据挖掘及理论学习。方法:对近10年针灸治疗慢性心力衰竭的文献进行检索,选出有效文献41篇。采用古今医案云平台(V2.2.3)和SPSS 23.0、SPSS Modeler 18.0分析临床选穴的配伍规律。结果:数据分析结果显示,腧穴总个数为59个,总频次276次,频次在5次以上的腧穴有20个,位列频次前5位的分别为内关、足三里、神门、血海、心俞。涉及经脉15条,手厥阴心包经的应用最多,其次为任脉、足太阳膀胱经;选穴配伍方面多采用内关-膻中、内关-神门、内关-血海、内关-心俞。结论:总结针灸治疗慢性心力衰竭的腧穴配伍规律,在临床施治中据此规律将内关、足三里、神门、血海、心俞作为主要穴位并随证加减,有利于发挥各穴位协同效应,提高治疗效果,促进针灸的发展。

Objective:To search the literature of acupuncture and moxibustion treatment of chronic heart failure in the past ten years,analyze the characteristics and rules of clinical acupoint compatibility,and provide data basis and theoretical support for the selection of acupuncture points of modern clinical acupuncture for the treatment of chronic heart failure.Methods:The literature on acupuncture and moxibustion treatment of chronic heart failure in the past ten years was searched,and 41 effective literatures were selected.The ancient and modern medical case cloud platform(V2.2.3),SPSS 23.0,and SPSS Modeler 18 were used to analyze the compatibility law of clinically selected acupoints.Results:The results were analyzed by data.The total number is 59,with a total frequency of 276 times.There are 20 acupoints with a frequency of more than 5 times.The top 5 used frequencies are Neiguan,Zusanli,Shenmen,Xuehai and Xinshu.15 meridians are involved,and the pericardial meridian of the hand Jueyin is the most used,followed by the Renmai and the bladder meridian of the foot;The selection of acupoint compatibility is mostly Neiguan-Tanzhong,Neiguan-Shenmen,Neiguan-Xuehai,Neiguan-Xinshu.Conclusion:After summarizing the law of compatibility of acupuncture points for acupuncture and moxibustion treatment of chronic heart failure,and according to this law,Neiguan,Zusanli,Shenmen,Xuehai and Xinshu are used as the main acupuncture points.The synergy of acupuncture points can improve the actual effect and promote the application and development of acupuncture.
