“怪三针”联合柴胡加龙骨牡蛎汤治疗脑梗死后失眠临床观察

Clinical Observation of “Three Strange Needles” Combined with Bupleurum Plus Osdraconis and Oyster’s Shell Decoction in the Treatment of Insomnia after Cerebral Infarction

目的:观察"怪三针"联合柴胡加龙骨牡蛎汤治疗脑梗死后失眠的疗效,并探讨其作用机制。方法:58例脑梗死后失眠患者随机分为对照组及观察组,每组29例。观察组给予"怪三针"联合柴胡加龙骨牡蛎汤干预治疗,对照组给予口服艾司唑仑片。观察比较两组患者治疗前、治疗结束时及治疗结束后4周时匹兹堡睡眠质量指数(PSQI),比较两组治疗后的近期疗效及远期疗效。观察比较两组患者治疗前与治疗结束时汉密尔顿抑郁量表(HAMD)评分,比较两组治疗前后的不良情绪情况。结果:与治疗前比较,治疗结束时各组PSQI、HAMD评分均明显下降(P<0.05);与治疗结束时比较,治疗后4周观察组睡眠情况进一步改善(P<0.05),对照组睡眠情况则明显反复(P<0.05);观察组的近期及远期临床总有效率均明显优于对照组(P<0.05)。结论:"怪三针"联合柴胡加龙骨牡蛎汤能有效改善梗死后失眠患者的睡眠质量,具有较好的近期及远期疗效,且可改善患者的不良情绪。

Objective:To observe the therapeutic effect of"three strange needles"combined with Bupleurum Plus Osdraconis and Oyster’s Shell decoction in the treatment of insomnia after cerebral infarction,and to explore its mechanism.Methods:58 patients with insomnia after cerebral infarction were randomly divided into control group and observation group,29 patients in each group.The observation group was treated with"three strange needles"combined with Bupleurum Plus Osdraconis and Oyster’s Shell decoction,while the control group was given oral Estazolam tablets.Pittsburgh sleep quality index(PSQI)was observed and compared before treatment,at the end of treatment and 4 weeks after treatment.The short-term and long-term effects of the two groups were compared.At the same time,Hamilton Depression Scale(HAMD)scores before and at the end of treatment were observed and compared,and the adverse mood before and after treatment was compared between the two groups.Results:Compared with before treatment,PSQI and HAMD scores of each group were significantly decreased at the end of treatment(P<0.05);Compared with that at the end of treatment,the sleep situation of observation group was further improved4 weeks after treatment(P<0.05),the control group was significantly repeated(P<0.05).The short-term and long-term total clinical efficiency of the observation group was significantly better than that of the control group(P<0.05).Conclusion:"Three strange needles"combined with Bupleurum Plus Osdraconis and Oyster’s Shell decoction can effectively improve the sleep quality of insomnia patients after cerebral infarction,with good short-term and long-term effects,and can improve the patients’bad mood.
