美洛昔康联合中药贴敷对腰椎间盘突出症术后康复疗效研究

Effect of Meloxicam Combined with Chinese Medicine Application on Postoperative Rehabilitation of Lumbar Disc Herniation

目的:观察美洛昔康联合中药贴敷对腰椎间盘突出症术后的康复疗效。方法:将90例行腰椎间盘突出症术患者随机分为对照组和观察组各45例。对照组口服美洛昔康,观察组给予美洛昔康联合中药贴敷,疗程均为3个月。观察治疗前后两组日本骨科学会腰痛评分(JOA),Oswestry腰背及下肢功能障碍指数(ODI),血清疼痛介质[前列腺素E2(PGE2)、内皮素-1(ET-1)],炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]。结果:观察组治疗后总有效率97.8%(44/45),明显高于对照组的82.2%(37/45)(P<0.05)。与对照组治疗后比较,观察组ODI明显降低(P<0.05),JOA明显升高(P<0.05)。与对照组治疗后比较,观察组疼痛介质(PGE2、ET-1),炎性因子(TNF-α、IL-6)明显降低(P<0.05)。结论:美洛昔康联合中药贴敷可明显提高腰椎间盘突出症术后康复的效果,其作用机制可能与降低疼痛介质与炎性因子有关。

Objective:To observe the effect of meloxicam combined with Chinese medicine application on postoperative rehabilitation of lumbar disc herniation.Methods:90 cases with lumbar disc herniation were randomly divided into control group and observation group,45 cases each group.The control group took meloxicam orally,and the observation group was given meloxicam combined with Chinese medicine application for 3 months.The Japanese Orthopaedic Society low back pain score(JOA)and Oswestry low back and lower limb dysfunction index(ODI)were observed before and after treatment.The serum pain mediators(prostaglandin E2(PGE2),endostin-1(ET-1))and inflammatory factors(tumor necrosis factor(TNF-α),interleukin-6(IL-6))were detected before and after treatment.Results:The total effective rate was 97.8%(44/45)in the observation group was significantly higher than that 82.2%(37/45)in the control group(P<0.05).Compared with the control group after treatment,ODI in the observation group was significantly reduced(P<0.05),and JOA was significantly increased(P<0.05).Compared with the control group after treatment,the pain mediators(PGE2,ET-1)and inflammatory factors(TNF-α,IL-6)in the observation group were significantly reduced(P<0.05).Conclusion:Meloxicam combined with Chinese medicine application can obviously improve the postoperative rehabilitation effect of lumbar disc herniation,the mechanism of action may be related to the reduction of pain mediators and inflammatory factors.
