内外推拿法结合关节松动术治疗颞下颌关节紊乱疗效分析

Analysis of Clinical Effect of Internal and External Tuina Combined with Joint Mobilization on Temporomandibular Disorders

目的:探讨内外推拿法结合关节松动术对颞下颌关节紊乱病的疗效。方法:随机将76例患者分成采用内外推拿法结合关节松动术治疗的观察组及采用常规推拿治疗的对照组,每组各38例。经过10次治疗后,评价两组患者视觉模拟评分(VAS)、最大主动开口度(MMO)及整体疗效。结果:治疗前两组VAS评分比较差异无统计学意义(P>0.05),具有可比性,但MMO指标有统计学差异(P<0.05),而治疗后均无统计学差异(P>0.05)。两组治疗后VAS评分比较,差异有统计学意义(P<0.05);MMO指标比较,差异有显著统计学意义(P<0.01)。两组治疗前后组内各数据相比,差异均有显著统计学差异(P<0.01);观察组的总有效率为97.37%,高于对照组的总有效率(94.74%),但差异无统计学意义(P>0.05)。结论:推拿可改善颞下颌关节紊乱所致疼痛、张口受限等症状,但内外推拿法结合关节松动术治疗更全面,疗效更佳。

Objective:To explore the clinical effect of Internal and External Tuina combined with Joint Mobilization in the treatment of Temporomandibular Disorders.Methods:76 Temporomandibular Disorders patients were divided into two groups randomly.The patients of the observation group,which had 38 cases,were treated with the therapy that was Internal and External Tuina combined with Joint Mobilization.And the other 38 cases were treated with conventional Tuina.Before the first time of treatment and after 10 times therapy were finished,the changes of the scores of the Visual Analogue Scale(VAS)and the Maximum Active Mouth Opening(MMO)of the patients were compared.Results:Before treatment,the difference between the two groups’VAS scores was not significant(P>0.05),which means they were comparable.But there was significant difference of the two groups’MMO(P<0.05).After treatment,both the differences of VAS scores and MMO between the two groups were not significant(P>0.05);but these data changed significantly compared with before(P<0.01).The difference of the changes of the VAS scores between these two groups was significant(P<0.05),and it was more obvious between them in MMO(P<0.01).After all,the total efficiency of the observation group was 97.37%,significantly higher than 94.74%of the control group(P<0.05).Conclusion:Tuina could release the pain of Temporomandibular Disorders patients and improve their mouth opening limited,and Internal and External Tuina combined with Joint Mobilization could make treatment more comprehensive,and showed better effect.
