摘要
目的:通过基于体素的形态学(VBM)和静息态能磁共振成像(rs-fMRI)研究为期5周的减重干预对女性肥胖患者的大脑结构与功能连接的影响。方法:通过饮食控制对16名受试者进行为期5周的干预,分别于干预前后对其进行脑区3.0T磁共振扫描。使用SPM12对数据进行预处理与分析,采用配对样本t检验进行统计分析。结果:减重后患者的体重指数等生理指标显著改善(P<0.05),左侧脑岛的灰质体积下降,右侧小脑髓质、钩回的灰质体积有所增加,且中央内侧和旁扣带回、左侧背外侧额上回、右侧中央前回和中央后回与左侧脑岛的功能连接升高。结论:饮食控制减重法可以一定程度上减轻患者的灰质萎缩伤害,抑制患者胃肠道感觉或恢复味嗅觉异常加工以达到显著的减重疗效。
Purpose:To study the effect of weight loss on brain structure and functional connectivity in female obese patients,voxel-based morphology(VBM)and resting state functional magnetic resonance imaging(rs-fMRI)were performed.Methods:Sixteen study participants were intervened by diet control for 5 weeks.And the 3.0 T structural brain MRI measurements for each participant were performed before and after the intervention.The data were preprocessed and analyzed by SPM12,and the paired sample t-test was used for statistical analysis.Results:After intervention,the body mass index and other physiological indexes were significantly improved(P<0.05),the gray matter volume of left insula(ROI 1)decreased,the right cerebellar medulla(ROI 2)and uncus(ROI 3)increased,and the functional connectivity between middle cingulate gyrus,left dorsolateral superior frontal gyrus,right precentral gyrus,postcentral gyrus and left insula(ROI 1)increased.Conclusion:The therapy of diet control for weight loss leads to the gray matter atrophy injury of patients to a certain extent,inhibits the abnormal processing of gastrointestinal sensation or restores taste and smell,so as to achieve significant weight loss effect.
