冠状动脉左主干病变的临床特征及其危险因素分析

Clinical characteristics and risk factors of left main coronary artery lesions

目的探讨冠状动脉左主干病变的临床特征及其危险因素。方法选取该院收治的358例冠心病患者为研究对象,依据是否存在左主干病变分为左主干病变组87例和非左主干病变组271例。比较两组患者临床资料、辅助检查指标的差异,并采用多因素回归分析对左主干病变的危险因素进行评估。结果左主干病变组年龄、有糖尿病病史比例和尿酸水平均显著大于非左主干病变组(P<0.01),两组之间其余指标差异均无统计学意义(P>0.05)。经多因素Logistic回归分析显示,年龄、糖尿病病史和尿酸水平均是导致左主干病变的危险因素(P<0.01)。结论冠状动脉左主干病变患者的严重程度更高,年龄较大、有糖尿病病史、血尿酸水平升高是左主干病变的危险因素。

【Objective】To investigate the clinical characteristics and risk factors of left main coronary artery lesions.【Methods】A total of 358 patients with coronary heart disease in our hospital were selected as the research object.According to the presence of left main lesions,they were divided into left main lesions group(87 cases)and non-left main lesions group(271 cases).The differences in clinical data and auxiliary examination indexes between the two groups were compared,and the risk factors of left main lesions were evaluated by multivariate regression analysis.【Results】The age,proportion of diabetic history and uric acid level in left main lesion group were significantly higher than those in non-left main lesion group(P<0.01).There was no significant difference in other indicators between the two groups(P>0.05).Multivariate logistic regression analysis showed that age,history of diabetes mellitus and uric acid level were risk factors for left main lesion(P<0.01).【Conclusion】The severity of left main coronary artery disease may be higher.Older age,history of diabetes mellitus and elevated serum uric acid level are risk factors for left main coronary artery disease.
