背根神经节脉冲射频联合发散式冲击波治疗30例腰椎间盘突出症 被引量:1

Treatment of 30 cases of lumbar disc herniation by dorsal root ganglion pulsed radiofrequency combined with divergent shock wave

摘要 [背景]观察背根神经节脉冲射频联合发散式冲击波治疗腰椎间盘突出症(LDH)的效果.[病例报告]选择60例LDH患者随机分为A组和B组,每组各为30例,其中A组给予背根神经节脉冲射频治疗,B组给予背根神经节脉冲射频联合发散式冲击波治疗.采用NRS评分、直腿抬高角度及JOA腰痛评分对两组患者疼痛情况及功能障碍进行评估.治疗后两组疼痛NRS评分、直腿抬高角度及JOA腰痛评分均较同组治疗前明显改善(P<0.05);治疗后B组1、3、6个月时的疼痛NRS评分、直腿抬高角度及JOA腰痛评分明显优于A组(P<0.05).[讨论]背根神经节脉冲射频联合发散式冲击波治疗LDH的镇痛效果及功能改善效果更为明显,且可有效治疗腰椎介入手术后的残余痛. BACKGROUND To observe the effect of pulsed radiofrequency combined with divergent shock wave in the treatment of lumbar disc herniation(LDH)in dorsal root ganglion.CASE REPORTS Sixty patients with LDH were randomly divided into group A and group B,with 30 cases in each group.Group A was treated with pulsed radiofrequency in dorsal root ganglion,and group B was treated with pulsed radiofrequency combined with divergent shock wave in dorsal root ganglion.NRS score,straight leg elevation angle and JOA low back pain score were used to evaluate the pain and dysfunction of the patients in the two groups.After treatment,the NRS score,straight leg elevation angle and JOA low back pain score of the two groups were significantly improved compared with those before treatment(P<0.05).The NRS score,straight leg elevation angle and JOA low back pain score of group B were significantly better than those of group A at 1,3 and 6 months after treatment(P<0.05).DISCUSSION Pulsed radiofrequency in dorsal root ganglion combined with divergent shock wave in the treatment of LDH has more obvious analgesic effect and functional improvement effect,and can effectively treat the residual pain after lumbar interventional surgery.
