不同输卵管因素对体外受精-胚胎移植的影响

Eddects of different tubal factors on in vitro fertilization embryo transfer

目的探讨输卵管积水、输卵管炎、输卵管梗阻3种输卵管因素对体外受精-胚胎移植(IVF-ET)结局的影响。方法回顾性分析江苏省徐州市中心医院生殖医学中心收治的首次接受IVF-ET的419例输卵管因素不孕患者的临床资料,并将其分为输卵管炎组(n=213,A组)、输卵管积水组(n=81,B组)和输卵管梗阻组(n=125,C组)。比较3组获卵数、受精率、卵裂率、高评分胚胎率、种植率、临床妊娠率、异位妊娠率及活产率。结果C组种植率、临床妊娠率高于A、B组,B组异位妊娠率高于A、C组,A组D3高评分胚胎率低于B、C组,A、B组活产率低于C组,差异均有统计学意义(P<0.05)。结论输卵管炎对IVF-ET的成功率有不良影响,在IVF-ET之前进行输卵管炎治疗十分必要。

Objective To investigate the effects of hydrosalpinx,salpingitis,and tubal occlusionon on clinical outcomes of in vitro fertilization embryo transfer(IVF-ET).Methods A retrospective analysis was performed for data of 419 infertility patients caused by tubal diseases who received IVF-ET for the first time in the reproductive medicine center of Xuzhou Central Hospital.They were divided into salpingitis group(213 cases,group A),hydrosalpinx group(81 cases,group B)and tubal occlusion group(125 cases,group C).The number of oocytes retrieved,fertilization rate,cleavage rate,high quality embryo rate,implantation rate,clinical pregnancy rate,ectopic pregnancy rate and live birth rate were compared among the three groups.Results The implantation rate and clinical pregnancy rate in the group C were significantly higher than those in groups A and B,the ectopic pregnancy rate in group B was significantly higher than that in groups A and C,the rate of D3 high-quality embryos in the group A was significantly lower than that in groups B and C,and live birth rates in groups A and B were significantly higner than those in groups(P<0.05).Conclusion Salpingitis has a negative effect on the success rate of IVF-ET,so it is necessary to treat salpingitis before IVF-ET.
